    In 2024 for esophageal pH monitoring some practices now have what is called the BRAVO capsule pH study, which gives you a DeMeester score to determine further management. The capsule is secured 5-6cm proximal to the lower esophageal sphinctor and disconnects on day 4 to be excreted via the bowels.

    Not for school or PANCE exams, but real life information regarding GERD: if your stomach acid is too LOW, the LES pressure sensitivity weakens and opens; so you actually want to increase the stomach acid. Counterintuitive, but it works.

      And that’s why most literature recommend PPI breaks and not taking it more than 4-8 weeks. Good point!!

    Hi!! thanks so much for this. I've been watching your channel for a few months and you've helped me a ton. I just had a quick question about treatment for eosinophilic esophagitis. On AMBOSS it seems the treatment includes PPIs and topical glucocorticoid. Which should I rather rely on? Thanks again :)

      Good point, it looks like the guidelines have changed a bit. So first line now includes PPI's combined with topical glucocorticoids (inhaled fluticasone or swallowed budesonide). Thank you!

    For infectious esophagitis, you skipped over the types of ulcers that hsv and cmv have. Also it’s blurred out on the slide in the video. Do you have an update for this?
