Here's my practical guide to melasma - what it is, how to treat it and ways to stop it from coming back so you can get back to enjoying summer.
Hyperpigmentation is one of the commonest complaints I see in clinic - and melasma is perhaps the biggest challenge. This results in hard-to-conceal brown/grey splodges in the centre of the face and its more common in women with deeply pigmented skin who are exposed to UV, often in the context of pregnancy or oral contraceptives. It often requires a prescription skincare approach; but if its mild, it’s worth considering trying these over-the-counter measures to bring it under control.

Very informative video. Thank you Dr. Sam! My mother has been struggling with melasma since my younger sister's birth (20 years!). She tried hydroquinone for years, to no avail. Over the years, it has affected her confidence unfortunately. I found a cosmetic dermatologist who administered a chemical peel and gave her an aha night cream and a mineral sunscreen. It's been a couple months but I'm praying she sees an improvement. I'll look into the Glow Therapeutics cream as well.

Thanks for the vid doc. I am a male and got hit with melasma last week while on a cycle of Doxycycline after having my wisdom teeth pulled. It's really depressing but I've started a strict regimen sunscreen, retin-a and hydroquinone 4%. I'm also going to start doing at-home 40% lactic peels. Wish me luck.

I have rosacea and I believe my skin is highly reactive because of it (can't even tolerate La Roche Posay or even Cetaphil gentle moisturizers or cleansers; my skin gets hot and bright red). I am new to your channel and love it already. Dr Sam, i have long been a user of Benzoyl Peroxide and Retin -A as I suffered from acne as teenager. But lately, i have been loving and using a lot of Asian products ( essences, toner and ampules). Would these ge as effective as Retin A to keep wrinkles at bay? i do love the pampering effect of these Asian products but i do know Tretinoin and Benzoyl Peroxide have done wonders on my skin. May i use these products on alternate nights to get the pampering effect and the prescription results?

Thnx for such a helpful video +Dr.Sam Bunting..I'm from India and used to hav great skin.I underwent radiofrequency treatment 6months Bck..after the treatment I suffered from v.dry and sensitive skin and had inflammation,my pores opened up.my Derma recommended squalene based moisturizer dt helped replenish my skin.now I hv hyperpigmentation marks on my facial skin area which ws inflammed.kojic acid based creams didn't helped me.I even got tanned as I didn't applied sunscreen for a month after d treatment.. currently I hv pigmentation wd uneven skin tone and enlarged pores.Will try the bioderma range products..Wats ur valuable suggestion..
Have had a fairly good result with Sunday Riley Luna and Good Genes alternating with Alpha H Liquid Gold. I've also heard that there are studies on using anticoagulant meds that have been shown to have results not sure if this is true?

No. Lemon can irritate skin and any irritation to skin make either cause or make melasma worse.
I have malasma. I got spotchy after a really bad burn. I'm using sun block, and sunscreen as well as otc retinol, vitamin c, niacinamide, licorice, aha, hydroquinone 2%, all with decent results. I also developed a rash when I got burnt, in all areas that were exposed to the sun. I now get this rash in the spring, every year for the past 3 years, and it doesn't go away til late fall. I use hydrocortisone cream with some effect, as it stops the burning and mild to moderate itching, as well as takes some of the redness away, and makes the spots smaller and flatter and stops it from spreading. I do take gabapentin, flexeral, and receive botox injections for nerve damage, hyperspasms, and cervical dystonia. I heard that certain medications can make a person photosensitive, and I was on these medications at the time I was burned. the sunburn I received blistered and was red and peeling and I felt I'll at the time I had the burn. so it was a bad burn. I was wondering what I can do for this and what exactly it is I'm dealing with

Nurjk Agloni Try Melascreen by Ducray. My dermathologist prescribed it to me and it has about 12% azelaic acid.

I looked up the heliocare supplement and it says it’s high in vitamin d. I’m on prescription vitamin d for low levels (20’000 units summer 40’000 units winter). Could I still take these or would I overdose on vitamin d?

I’ve got melasma before I got pregnant, worse after. I use vit C, retinol & other its help immensely to fade my melasma and other hyperpigmentation. Also on my lips. It’s taken me 1 year. Should I look for a stronger/active form of vit c and retinol? I use Osmosis product.
Thank you for the video. Subscribe. I had a pico laser on my left cheek last month and then a week after, I did a follow-up with the dermatologist. She who noticed the skin was peeling but it was red and she was concerned that it might darken later. Well it did.The pico laser didn't help remove my sunspot on my cheek. I'm not happy about it one bit. So disappointed. Did the dermatologist mess up? Does it take more than 1 try? I'm supposed to follow up with the dermatologist a month after the procedure. I don't know if I want to see her again. I'm afraid of doing another laser. Is there any topic cream to remove it? Obagi? Others? I've used it years ago. Thank you for your help.
Lasers are NOT good for melasma - they either don't help or make the situation worse. Chemical peels seem to be way better. People, please don't listen to people that tell you laserotherapy will help you - it's called marketing and sales.

doctor i got melasma from laser resurfacing its all over my face .. what should i do , should i wait it might go on it own . or should i treat it ... i dont want to use bleaching creams im allergic ?? i have tried lactic acid / tca .. now im trying mandelic acid .. but nothing seems to be working .. every dermatologist i visit they prescribe to me hydroquinone im allergic to it ... what to do my face looks very gray and brown i have skin type 3 please help .....
Don't look to MDs to cure you... diseases are their cash cow so they will only "treat" you ad infinitum with prescriptions, lasers, surgeries, etc. They will tell you how to superficially treat it, like this video, but melasma is caused by fungus which is an internal issue that can be cured. Seek out a good naturopathic doctor for true help. Good luck.

Every summer I get what looks like melasma across my chest around the armpit area on my front and also down one arm. it tends to looks like patchy self tan gone wrong, but it disappears in a winter. Is this melasma too? does the same kind of treatment apply for such a large area on the body? Thanks.

Can the mirena cause melasma ?
Is there a sunscreen that doesn't burn? I haven't found one that doesn't make my face sting and burn all day. I love gardening, but I don't like burning chemicals on my face or my brown upper lip.
Commercial sunscreens are loaded with toxins that cause cancer. Make your own effective, pure sunscreen with coconut oil and zinc oxide, both you should be able to purchase reasonably at your local health store. Cheers
Christy T I use banana boat for baby's on my face 50 spf, mineral based, '"tear free" but you will look pale without makeup over top.

itsallgucci ya...i agree I'm all for diy's most of the time but putting coconut oil on your face and then going out into direct sunlight for a few hours is not one I'd consider when trying to avoid hyperpigmentation.
@itsblayne She's a dermatologist sk yes people are going to listen to her advice. Also, there's no proof that coconut oil helps protect skin. If anything it makes it worse
I have noticed my melasma progressively getting worse since going back on birth control. It is just on my cheeks. However I went back on birth control to control my acne since nothing else I was using before was working. Anything you can recommend or suggestions?

Hi, after my pregnancy I now have melasma across my forehead. I have been using Rationale skincare and have had 6 laser permea. I have seen a decrease in the melasma. The clinic I go to the nurses are extremely informative and now recommend a series of laser genesis. I would like to know you opinion on laser genesis to treat melasma?
I have not ever taking contraceptive pills, hormone supplements and didn't get it during pregnancy. So why do I have it? What should I do? If any cream out there clears it as soon as you go under the sun they come back. I used 50 spf or higher and stay away from the sun as much as possible. What should I do?

If you have access to esthetician services, I got amazing results from a combo of led light therapy and microdermabrasion. We did one treatment a week for six weeks, then follow up once a month for maintenance.
Also for at home treatment, use a retinol at night and a good vitamin C serum underneath 50 spf sunscreen during the day. The vitamin C treats melasma directly and indirectly by boosting the effects of your sunscreen (ZO by Obagi's C bright is a little pricey but by far the best C I've found.) A 2% or 4% hydroquinone serum will reduce the production of excess melanin, and can be used twice a day as a spot treatment, but should only be used on 4 month cycles (4 months on, 4 months off).
I'm from Trinidad, so far most doctors say,we get it from birth control pills, I don't believe that, I have never taken it, and I have malasma.
