Are you adding both the qualified leads conversion action, alongside the inquiry conversion actions and having 2 optimisation goals per campaign? Or removing the inquiry as a primary conversion from the campaign. As having 2 conversion actions killed my pmax performance before.
WIth the uploads. Do you only need to upload the leads that convert? Or isit beneficial to give $0 value to the leads that didn't too? Also, when uploading the Google sheet, do you need to remove the leads on there before you upload again with new data? Will Google spot the duplicate or double count the conversion?

No need to upload nonvaluable leads to the algorithm.
dont worry about the duplicate data Google doesn't count duplicate values, add new values under the existing values, it will be fine.
