    Greetings from Australia. Thank you so much for your videos and information. I needed a right great toe fusion in January 2024 and am now suffering knee and butt problems. I am doing exercises to strengthen quads , hamstrings and gluts, however I need exercises for calf muscles given the restrictions I now have to bend my big toe. Could you please advise me?

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  Месяц назад

      Hello Jane!! 🇦🇺🧡
      Sorry to hear about your great toe fusion...that's no fun! Yes, that will absolutely change how you move, especially with walking.
      We're glad to hear that you're exercising for the knee and butt problems!
      We don't have videos specific for your situation, but this one should help:
      "This Toe Trick Will Improve Your Knee Pain When Walking"
      ruclips.net/video/XTPTvkv-Diw/видео.html
      Be sure to follow the guidelines from your surgeon about how to bend it. If you're working with a physio on this, they should be aware of what your surgeon wants you to do. Most physios are great at helping people recover from surgery.

    this is one of the rare doctors and channels I've found that tells the truth. The ortho doctor I went to in my 40's for torn menisci w/out enough cartilage to "repair" them & severe knee osteoarthritis gave me wall squats & other quad exercises. When I asked about the severe pain from squats so bad I couldn't sleep, he gave another round of anti inflammatories & yelled "Keep doing the exercises!" I'm so grateful that almost 30 years later, I found this channel. Really glad they couldn't "repair" menisci & that the offers & pressure to get knee replacements, shots, over the decades, I turned down.

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  Месяц назад

      Wow, thanks for sharing your story Kathy!!! Sorry you were told to do wall squats and quad exercises before and then yelled about about not doing them even though they hurt so bad it kept you from sleeping :-(
      This happens more than we'd like to hear. But keep in mind that healthcare professionals usually have the best intentions and are doing the best they can with what they know. It sounds like you needed to see someone else.
      We're glad you dodged the meniscus repair surgery and knee replacement!
      Get going on those glute exercises to keep your knees healthy!!!

    I've never been able to do wall squats since developing severe osteoarthritis. Regular squats with or without weight cause a lot less pain. I thought there was something about my bone structure that prevented me from doing them. Either way, I don't even bother with them now and focus mostly on glute strengthening exercises. Thank you for setting the record straight on these types of squats.

    IMO. All joint issues are from muscle imbalances and bio mechanical asymmetries. I see these asymmetries all the time. A biggie is the runner who spends a split second longer on one foot than the other. Very noticeable if you look for it. But these asymmetries are everywhere and if they were addressed I bet the bulk of hip and knee replacements would be unnecessary. We need more physiologists.

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  Месяц назад

      So true Scott!
      Runners can do about 2,000 steps every mile...that means thousands of harmful reps every run!
      This is exactly how muscle imbalances work - they go undetected to most people (even healthcare professionals) because they are so subtle.
      Then after thousands of moments of bad pressure people start to develop pain.
      Keep this up for years or decades and it eventually shows up as worn down cartilage and bone-on-bone knee arthritis.
      This is why this channel exists!!! If we can fix it early then most joint replacement surgeries would be unnecessary!!!

    Not for everyone as soon as I added wallsits for quad strengthening for pfps rehab it started to improve. Just dont do quad work in positions that hurt. Did glutes for a long time and they got very strong no improvement once working quads it improves. So most likely depends if your quads are stronger then glutes or much weaker.

      Hey @yakncast7530, absolutely we know about Ben the Knees Over Toes guy.
      We're not familiar with the details of his protocol.
      But our best advice for choosing what to do to fix knee pain is to try it and see how your body responds.
      If it works, then great!
      If it hurts, then also great because you can cross it off the list and try the next thing.
      You don't win or lose, you win or learn.
      Ultimately you need to figure out what you need to do for yourself.

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  Месяц назад

      Hey Julie! Great question on Romanian Deadlifts and knee arthritis.
      It ultimately depends on how well you can use your glutes during the exercise. Some people do it better than others. It has to do with your genetics as well as how your body is used to activating and utilizing your muscles during the exercise.
      If you cannot use your glutes while doing the exercise then it would not be good for you because you'll be compensating with other muscles. Usually hamstrings and lower back muscles.
      If you feel your glutes activating more than anything else during this exercise, then this is a great exercise for you.

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  Месяц назад +1

      Hey Assama! Sorry to hear that your coccyx pain gets worse when doing knee exercises :-(
      That's a sign that you might need to re-think your strategy.
      It could be that you need to see a specialist about the coccyx pain to make sure there isn't a fracture or shifted coccyx bone. No amount of exercise will correct this type of coccyx problem. You have to reduce sitting and use a cushion with a cut out for the coccyx when you do sit.
      It may also be a nerve problem from the lower back. Nerves come from the coccyx but may be pinched up higher in the lumbar spine.
      Then after those problems are cleared, we have to consider the muscles. Usually glute exercises, not knee exercises, are what is needed to best stabilize the coccyx.

    • @AssmaaHussien-kd1qs

      Hi Tony, We would never tell anyone to skip an exercise that feel is helpful to them. What we do what you to do is consider skipping it if you feel it is aggravating you. If you have knee arthritis and you do this exercise and it hurts we do recommend to refrain from doing it until your glutes have been strengthened enough to where the pain has subsided and or you feel confident that your glutes are strengthened.
