I still have problem with number 4. One of your videos you made us understood that, we have to look into the stem and the answers are mostly in the question. That question was similar to this. The patient reported his condition so per your explanation to the options, you emphasized that since the client has provided all the assessment, there's no need to assess the client again so the nurse should go on and administer the prescribed medication. What's the difference between that question and this number 4?
