I would be cautious with that last one, asking about work-life balance. A lot of hiring departments will instantly write you off as a lazy clock-watcher if you so much as hint that you have a life outside of work.
I think he included that because we should expect this out of companies. If you get the job, you will find out the hard way. If a company answers unfavorably, I will find another company to apply to.

All this advice is redundant. If a company is recruiting high flyers then they will be headhunted, and all these matters will arise organically during the selection process. For non executive levels the company will still be able to sift candidates through pre-interview stages as they will only be seeking "good enough" candidates that will be selected for promotion based on practical performance on the job. It's all become a nonsense now (cv for a cleaning job, zero hours for a non-director, extended interviewing for a receptionist role)
