Flandre looks so unhinged in the artwork, paired with the music it feels like she is enraged with her sister remilia for keeping her in the basement of the scarlet devil mansion for many, many years. Flandre is a destructive vampire and could take down remilia if she trained hard enough, but the reason Flandre won't able to take her down is that she has emotional control over Flandre and can guilt her into stopping the fight. Though remilia wouldn't hurt flandre badly, flandre's defeat would lead to even more hatred welling inside flandre, eventually leading to much more destruction and damage to the mansion. Remilia then would have no choice but to take harder precautions to keep flandre contained within the mansion, maybe even having her end flandre in serious situations. That is my explanation for flandre's insanity!
