If you want to see a more accurate representation of what Die Ärzte concerts look like (in terms of audience), I'd recommend to check out videos of "Die Band, die sie Pferd nannten" or "Die Nacht der Dämonen". Those were recordings/DVD releases of Die Ärzte concerts from 2003 respective 2012. (One of the two concerts that were recorded for "Die Band, die sie Pferd nannten" was my first ever Die Ärzte concert. Rock am Ring is an event for a weakend presenting several bands. So the large crowd might not consist of "Ärzte" fans only. It is a rock concert and I think I am correct to call them rock fans.
You are right that this was a festival with several rock bands, but I can assure you: they are so popular! They can easily draw such large crowds to their own concerts. And they have proven this multiple times :)
I saw them at Rock am Ring at the beggining of the month, and people were clearly here for them. People were waiting at the stage way earlier than for the other bands, and they were also a lot more people.

I saw them on sunday. They played 3 days in a row in Berlin, everyday there were 60.000 people Oo ❤
