I was hoping that you would give the inside thread a good squirt with brake cleaner before you applied loctite to the cleaned bolt.

No o-ring between the piston and the rod? That's a really poor design.
Suggestion for the future. Use wood dowel or hard plastic to hammer on the aluminum piston and/or gland. Brass is harder than aluminum
