I learned multiple versions of many techniques. I pick one to teach as a base and then teach the many applications as my students get more advanced.

Is combination 12 a front ball kick with your left foot and a spinning back kick with your right foot?

Against a right side attack, Yes.
As the right hip moves forward you stop it with your left side, it's the mirror image side instead of crossing your body with your right foot.
It can work the other way, but is more efficient this way.
