It is both government and people's fault. Government has never set up the strict rules and provided the knowledge to people about the eco system, proper health and hygiene which plays enormous rolls on environment and everyday lives. Most Nepali government bodies and also the people are uneducated and ignorant about the clean and healthy environment. It needs massive work to enlighten these folks about sanitation and clean environment.
Hi Ken For your next visit, as you have Pokhara already on your list, you could visit Mustang which is near Pokhara. Mustang is a Zielort for easy and difficult hiking. LOWER Mustang for easier and UPPER Mustang for difficult. Then, you'd get a village experience as well. There is a Tibetan Settlement there which you must visit. They produce Apple juice from fresh apples in their orchards - thank me later for this suggestion 😊 Whilst in Pokhara go up to Sarangkot from where you can see the Annapurna mountain range! You will be enchanted 😃
Kathmandu is chaotic yet beautiful in its own way. I was born in Kathmandu but left Nepal when I was quite young, and yet it amazes me how much i still need to learn about it! Every part of Nepal has its own charm and beauty. We do have flaws being a developing country and all that but people are generally nicer and peaceful ( need to careful of street vendors everywhere!! I had the worst experience in New York with the street vendors which shows that such people are everywhere lol) but hopefully you get to travel outside of Kathmandu, explore the outskirts! There are mountains that can be hiked, and valleys which are heavenly, many say it’s heaven on earth(need to find that one out myself) but hopefully you get to see all of that next-time. Regarding trash. I live in a beautiful small village in Thailand and it is starting to look dirty with people dropping plastic rubbish everywhere. There is also no refuge collection so the rubbish and plastic is burned. It's a real shame.
For trekking you need separate man holding camera and your bag luggage. Walking with camera and talking on camera equally is not easy like walking around and video in Kathmandu.

Before modern wastes and plastic, every thrash was biodegradable and helped to improve fertility of soil. So it was never taken as a problem but after plastics and modern undegradable wastes, people weren't and still aren't alert enough about the consequences

Be very careful of street food in Nepal. Tourists can't handle street food of nepal. Proper hygiene is not maintained and if you eat anything thats out in the open then poisoning is kinda gauranteed
