What the old man is talking about is not a wall break he's actually aknowledging the lore of the series and posibly giving foreshadowing to a real world separate from the cycle

The Rivage Elder is actually talking about the unmoored world (not our real world).
In the unmoored world, if you die and don't use a wokestone to revive yourself, you will die, and a cutscene will play out where it turns out that you are the rivage elder.
This basically means that the rivage elder was an arisen who got to the unmoored world, but died, and wasn't able to defeat the pathfinder.
