Giyu shouldn’t have defence because his defensive ability (dead calm) was broken through by Akaza because he was too fast so I’m sure Douma wouldn’t have a problem doing so as well. Also Douma can create as many clones as he wants which I’m pretty sure can all use his bda and the only reason they weren’t really helpful in the manga is because he was poisoned.

The main reason Douma is ranked higher than Akaza is that when they faught eachother, Akaza couldn’t detect Douma's Battle spirit. Detecting battle spirit is Akaza's main power. But Douma is a phsyco. Even while fighting he's always smiling and talking. That's why he defeated Akaza. But Akaza is stronger in terms of killing Demon Slayers. Cause Akaza has more than 100 years of more experience than Douma. Akaza's regeneration speed is faster than Douma's. And Just Like the Demon king and Kokushibo, Akaza is also Immune to beheading. So overall Akaza is Much harder to kill alone. On the other hand Douma's BDA is the best. But his lung freezing ability is only effective on weak slayers. Like it did harm Shinobu but it couldn't do sh!t to Inosuke. And the Power gap between Inosuke and Giyuu is Leagues away. And in the upper 3 fight, Tanjiro got Knocked out right away. He couldn't even stand Against Akaza before he unlocked selfless state. He was day dreaming about his father and how to unlock selfless state. Giyuu on the other hand did get blown away by Akaza's Kick wich is like a thousands of kicks flying twords the opponent at lightning speed. So i don't think any other pillar(exept Yoriichi) could evade an attack like that. Mostly because it was a very Close range attack Giyuu didn't even hame to evade it. But After that, Giyuu was the one who carried the whole fight. Tanjiro and Rengoku were not being able to keep up with Akaza's base speed. While Akaza needed a speed boost to match Giyuu's speed. Still Giyuu managed to Strike at Akaza's neck several times.But even Giyuu did cut Akaza's head completely it would be no use. Cause Unlicke Douma Akaza Won't die even after his head is cut off. But Giyuu couldn’t unlock selfless state so no matter how fast he attacked Akaza could detect it. So the faster Giyuu got the faster Akaza got. And Akaza could only break his sword when Giyuu got exhausted after a long blow to blow fight. But luckily Tanjiro managed to unlock selfless state he could behead Akaza. Cause Akaza couldn't detect it. And Giyuu also Faught Muzan after that. Not to mention he faught with a broken sword in the beginning, and despite of that he did great. Though Obanai was the best performer in that fight Giyuu and Sanemi also did great. And even after fighting Muzan he didn't die. He also took damage from DKT he still survived. So If Giyuu faught Douma alone, Yeah Douma would win. But no way in Low-diff. Even if Douma uses 100% of his power it wouldn't be low-diff. And if Giyuu somehow does manage to slice Douma's neck, Douma is dead. Cause keep in mind that Muzan, Kokushibo and Akaza are the only demons that can still fight after loosing heads and regenerate it again. But Giyuu still can't win alone cause all of Doumas's clones, Ice states will exhaust Giyuu and since he is a human his stamina won't last forever and at one point he will get exhausted and Douma will mange to kill him. But as long as Giyuu has stamina, Douma will have his a$$ jumping. Cause Douma can't detect his slashes. And in a blow to blow combat, Giyuu's Sword wielding speed is insane. But Giyuu can only kill Douma alone if he manage to turn his blade red. Wich will make Douma's Regeneration speed slower than a lower moon demon. And ofcourse Giyuu won't just stand there and watch Douma regenerate. He will instantly slash his neck. But since Giyuu couldn’t turn his blade red against Akaza, same will go for Douma. Douma will win. But not Low-diff. It would be an insane fight though.
