Bro my tank water gets yellow in 5 days i use canister filter( ceramic ring bio media and activated carbon) and not is overstocked is there any suggestion for this?

What should I do to the turtle because he is not eating and blood came from his mouth when he fell down. Plz reply fast😭. He also became very weak after falling down and he is not moving also he is keeping his head up, he is not dipping his head in water
