I know this comment is old now, but it's likely because of the use of topical anaesthesia being regulated, which Des mentions in the video (but it's very easy to miss, in the middle of a back and forth, very quick). More people seem to find needling painful. I do not, however, and know that I am far from alone. It should still be possible to do a lot of patients at 0.5mm depth without any anaesthetic, but I'm not sure how one gauges who is going to be fine with that and who isn't if they're not a return client.
