    Epididymo-orchitis is an inflammation of the epididymis and/or testicle (testis). It is usually due to infection, most commonly from a urine infection or a sexually transmitted infection. A course of antibiotic medicine will usually clear the infection. Full recovery is usual.
    Causes of epididymitis
    Epididymitis can be caused by a sexually transmitted infection (STI), such as chlamydia or gonorrhoea. This is more likely in younger men under 35 years old.
    It can also be caused by a urinary tract infection (UTI), usually in older men, but UTIs are less common in men. If its due to a UTI then you must have a review by a urologist.
    A UTI is more likely if you have:
    an enlarged prostate gland
    a urinary catheter
    recently had surgery to the groin, prostate gland or bladder
    Sometimes a cause cannot be found.
    You can also get treatment for epididymitis at a sexual health clinic.
    They are used to diagnosing, investigating and treating EO and can also provide you with the correct antibiotics. In countries where sexual health is not a medical specialty then family doctors or urologists with an interest in sexual health are a good alternative.
    Remember it can take months to fully recover.
    Have regular sexual health checks and have good sexual health

    what kind kind of antibiotic do use for this. iv bad 2 diff kind s so fare . none works . although the first time I had doc give a shot then 10 days of pills. the shot workd completely took everything away. days later it came back

    Dr Bret Palmer, is it ok if I take my open my doxycycline capsule and eat in with a tiny bit of apple sauce and water in 1 quick gulp? I have a problem swalloing pills and was told this was ok by a couple pharmacists.

      To be honest, I don't know. However, you can get dispersible doxycycline tablets, which dissolve in about 10 mins, this is what we try to give if someone can't swallow.

    @DrBretPalmer, why would someone who has chronic or recurring epididymis be prescribed Tamsulosin or Flomax as a treatment even if their prostrate is fine?

      Hi there Avneet,
      It's all in the video.
      Guidelines advise Doxycycline 100mg twice a day for 2 weeks plus an injection of ceftriaxone 1 gram.
      Or
      Moxifloxacin 400mg Once a day for 14days. No heavy exercise, risk of tendinitis.
      Other antibiotics can also be given as well, depending on the cause. All antibiotics should be given by a doctor after a full medical review.

    Thanks for the content. This is day 7 for me on Doxy after being diagnosed with EO. I was only given a 10 day supply (USA). While the pain has gone away, the feel/appearance has not all the way improved, perhaps only a little bit. I've been doing everything in my power to bolster the healing process, i.e. laying in a position where my scrotum is above my heart, sitting in warm bath, taking it easy overall, lots of fluids, supportive bottoms, eating more garlic and other healthy foods, etc. Is there anything else you would suggest? Not being active is killing me so would really like this resolve itself in the next week or so.

      Thank you for the question.
      Supportive underwear, like briefs can help.
      You can be active, just Need to do the right sports, swimming supports the weight of the genital running and cycling will upset them.
      General soreness can last a few weeks e.g. 6. So don't set a time limit on the body to get better.
      If you get worse after your treatment then you will need to test for mycoplasma genitalium if not already done so.
      Also get your sexual partners tested and treated with 1 week of doxycycline 100mg twice a day.

    SIR
    I AM 34, MALE, I was diagnosed with EO 6 years back. At that time I had swelling in Right Testis and dull pain. I did the antibiotics Course but swelling didn't go away completely. At that time.. Doctor told no further RX. There is still a bit swelling in my Right Testicle after 6 years. I do feel occasional dull pain in my right testicle. But it goes away on its own. What could be the treatment for this swelling ?? What could be the possible complications which can occur if I leave it untreated. ?? Please guide.

      Long standing swelling is difficult to halt. Depends on the cause. Usually supportive underwear may help reduce the swelling and an USS may be helping in identifying a cause if not know.

    Hello sir
    ...dose this infection epididymitis orchitis damage the microtubes from the epididymis? it seems nobody talks about this posible trauma after the infection is over especially when it was large swollen this can tear apart every thing inside the epididyme and make someone infertyle ?
    This is very scary ans serious condition hope everybody gets out well from this

      Yes EO increases the chances of infertility but the vast majority are absolutely fine.

    Can orchitis cause oligospermia or azoospermia ? And when treated can oligospermia or azoospermia be reversed

      Yes orchitis can lead to oligospermia and azoospermia, and asthenospermia (defects in sperm movement) especially if the orchitis is caused by mumps. However this is usually transient. If not then you may need to speak to a urologist.
      If you can masturbate and climax but don't ejaculate then you may have retrograde ejaculation which happens after infection of the prostate but something to think about.

      Hi there Pavnoor,
      Depends on the cause of inflammation.
      If bacterial then no.
      If due to trauma or injury then yes it can.
      Usually in the UK 2 weeks of doxycycline is usually given, but does depend on examination findings. Did you have a test for Chlamydia, gonorrhoea and mycoplasma genitalium?

    I have been taking doxycyline for 10 days now and symptoms such as tenderness, redness, scrotal sac size, lower abdomen tightness, frequent urination has gone away, however the size of my affected testicle is still almost the same (has maybe decreased slightly). Do most testicles go back to normal size within a couple weeks? Mine has been 2x the normal size for about a week. My tests confirmed I had a UTI and it has given me this epididymo-orchitis. Doctor is sending me for an ultrasound just to rule out testicle cancer but im guessing its reasonable to assume it isnt cancer because it all coincidentally happened when I got a UTI? He just seemed a bit concerned it was this size and kind of hard.

      Your GP is doing the correct scan. Sometimes, a pregnancy test can also be done, as some testicular cancers produce the beta-HCG hormone, which a pregnancy test can pick up.
      GP also wants to rule out a testicular haematoma as infection can cause these, but they rarely reduce.

    Hi dr, please can you help me. A month ago now I had a unprotected sexual encounter with a female. The day after I started experiencing symptoms such as a burning sensation during urination. I assumed I had an sti. I went to get tested 3 days after. Nothing showed up. They put me on a week of doxycycline. I went back a week later as I still had symptoms but nothing showed up still. During this week I started getting testicle pain and swelling and it became pretty painful. They were very tender and swollen. The testicles would actually become quite solid occasionally too. I called the GP and he considered that I might have epididytis/epididymal-orchitis and they have now put me on ciprofloxacin for 4 weeks. I’m 2 weeks into that treatment now and it’s barely any better if any better. I’m so stressed out and I don’t know what to do. I also get urine leakage as well. But at the same time my eurethra feels tight. Please can you advise.

      Hi MajorBen,
      Sorry to hear its not going well.
      It's been a few weeks after sex, so I'd advise to go back to the sexual health clinic or a different one.
      Don't pass urine for 2-3 hours before.
      Get a full screen for Chlamydia, gonorrhoea, HIV, syphilis and mycoplasma genitalium. If your clinic is small then they won't be able to investigate much... if thats the case, then approach your GP and request an Ultrasound Scan for the testicular pain.
      The sexual health clinic if testing for mycoplasma genitalium may give you treatment for this... Doxycycline followed by azithromycin or may go direct to 14 days of moxifloxacin. All depends on findings on slides and examination.

    Hi Doctor
    I had a bacterial infection which I was put on 2 weeks of doxy for about 2 months ago in that time the initial infection has cleared up however I still have swelling and inflammation in both my epididymis and testi. I feel no pain and swelling has slightly decreased since it started however only a slight discomfort soemtimes. I have also had an ultrasound which shows a very small cyst on the left. I would like to know how long it could take swelling to go down as I’ve only had this for 2.5 months I heard it can take up to 6 months but it’s really bothering me hoping it’s not chronic. I’m in the UK.

      Hi there.
      Are you using supportive underwear? E.g. briefs not boxers.
      Good the pain is gone.
      How noticeable is the swelling? If only slight and no pain, I'd wear briefs and should continue to reduce in time. USS, showed only a cyst which is good to hear.

      hi yes i wear underwear boxers that aren't too tight but are still quite loose they still cup and support my testis fine but i will look into wearing supportive breifs more. the swelling from the outside is not very noticeable however both testis still feel very hard and firm on the inside with slight swelling. however the swelling has gone down since it first started the firmness remains. as i mentioned i dont have much pain just slight discomfort sometimes and very minor aches from time to time. The doctor said everything was okay when looking at my ultrsound despite the two tiny cysts on my left testi. Any idea how long it would take for swelling and firmness to subside fully ?

      @@danialhaziq1533 no swelling has gone completely. I don’t have any pain anymore and everything is fine besides the 2 cysts it left me. For most or atleast for me it went away after 4-6 months. However it can last for a very long time.

    Can Microscopy found any bacteria from semen before proceeding to Culture?

      You can, but not always the best way to search for sexual infections.
      PCR testing is the best for Chlamydia, gonorrhoea and mycoplasma genitalium. Sperm testing and culture can help pick up other infections, microscopy can help but has a poor pick up rate compared to culture for non sexual infections.

      Hi Andy, thank you for your question.
      I'd recommend for epididymo-orchitis 2 weeks of doxycycline and consideration to also have ceftriaxone or azithromycin..... guidelines have changed again in the UK.
      1 week not the best for EO.
      If you live in the UK, then get a second opinion from a sexual health clinic. I'm assuming your treatment was given my a GP (family doctor), if you're from the USA, then get a review from a family planning Centre.

    Hi, Sir. I am 34. I have been suffering from Testicular swelling and pain past 35 days. I consulted a Surgeon and he told me to take Chymoral forte, Acemize-s, Oflox-OZ and Rantac -150mg. I have taken this for 30days. But there is no cure. After that I consulted a urologist and he told me to take Chymoral Forte and Doxyxycline . I have been taking this medicine from last 7 days. The swelling and pain remain same. In USG Scrotum , Urine culture AFB, USG KUB, Urine routine and micicroscopy test nothing found wrong. Please guide

      Hi there.
      Testicular swelling can be difficult to treat. Have you had an Ultrasound scan?
      If you have and is normal then what exactly is swollen.
      If obviously visibly swollen then why was the Ultrasound scan normal?
      Pain in the tesicles can be helped with supporting underwear. Anything that lifts the testicles to support and try not to do too many physical things for a few weeks, easy to say but very difficult to do as we all have to work.
      For testicular pain 14 days of doxycycline is usually given not 7.

    how long time swelling is gone and normal size ?
    i have this 2 week a go ,any bacteria can make this disease any Go/ Clamydia? because im not having sex after break up with my GF like a 3 Month and first day i got , i used antibiotics and 2 day after im used ciprofloxacin yellow discharge is going out and just once time , im so confused what happen to me
    i have pain urin 2 day and my testicel got bigger : (
    im used ciprofloxacin 10 day
    and now im used doxilycline 2 week
    - i dont go to doctor because covid , so scare to go hospital in condition now , whatever you problem doctor will say you got covid 😭

      If under 40 low risk of COVID.
      I think you do need a full check up on STIs so a full sexual health screen is needed.
      Pain can take 2-3 months to settle.
      If swelling not going with 2 weeks of doxy then USS and a pregnancy test should be done.

      Yes can be cured but wi need a full investigation.
      As well as sexually transmitted infections, you may need to get tested for testicular TB as well... does also depend on where you live, your medical and sexual history. You will need to be seen by a medic with experience in this area.

      Provided no allergies and reactions with other medications then doxycycline 100mg twice a day for 2 weeks with a full sexual health screen, should be fine.

      Depending on the cause, a correct antibiotic and support of the testicle will help cure epididymo-orchitis.
      You do need to see a medic who knows how to treat epididymo-orchitis correctly.

    Hello doc, igot EO last year. Never had sex. No UTI. Doc gave me levoflaxacin,chymoral plus for 2 months and flotral for 3-4 months. Its the worse thing i have ever faced.What are other causes of it and how can be prevented it from getting in future?

      Pain was initially worse so that intravenous injection of Amoxicillin was done for a week. It got better after 40-45 days.
      But perineal pain was there after. Doc suggested to get semen out once in two days after antibiotic dose. I felt releif only after that for sure

      @pschacker8270 sounds like you are back to normal. Prevention can be difficult. Sometimes TB can be a cause but not for you. If you have any tender areas then wear supportive briefs can help.
      For the future, wearing condoms for sex helps and drink enough fluids to keep your urine clear helps wash out bacteria.

    Dear doctor first i wanna thank you for helping People out Here youre awsome keep it up!
    I wanna share my situation with you,i woke up with a swollen and risen testicle,went to hospital for 7 days had fever each day was diagnosed epididymis orchitis they gave me vein antibiotics and my testicle swelling whic was 4x size went down to 2x they stopped with vein antibiotics for 2 days i had no fever and in the middle od night pain woke me up suddenly it was unbrarable i had to get a injection for pain i m not sure if infection returned or i accidently hurt my self,anyway NeXT morning they Sent me home i stay 1 day home and boom my testicle is 8x its size and i Got a fever again, i go back they do all test and ultrasound and keep me on hospital for 7 more days on New antibiotics on vein,after 2 days fever went away and my swelling reduced grom 8x bigger to 4x bigger it is hard and big but it doesent hurt ,i am home now for 7 days and my swelling is still the same should i be worryed they told me that it Will take alot of time for swelling to go away but still when i look at it i feel unease cause its big i keep thinking of bad things Like if they should let me go home again or not, i feel no pain when its suppprted or when i touch it from below Like it was before ,please give me any feedback abouth this swelling what can i do to reduce it is it normal that its Like 3x bigger,im supporting it and putting Water towels to keep it cooler,its red on the side big and hard,thank you for Reading this thank you for your time whoever read this and left any information god bless you
    (Extra details:they couldnt tell me what caused my condition because they tested me after they started filling me with antibiotics they are really not organised and im concerned of my swelling only pain o feel somethimes is in my place between leg and testicles from testicle Weight i support it)

      Thank you for sharing your story. You've had a difficult time. I don't know what the USS showed so if the swelling is due to blood then may not go down, if due to infection then should reduce.
      Do you know if they tested you for Chlamydia, gonorrhoea or mycoplasma genitalium? If not then get tested for these to rule them out.
      Do you have a follow up with the urologists? Then you will hopefully know what's going to happen for the future.

      Hi there, sorry for the late replay and thank you for your question.
      EO is a condition where bacteria (or other organisms) get into the wrong place at the wrong time, the bacteria do not have to be STIs, they can be any bacteria from the mouth, skin etc.
      After you treatment can take time for the soreness to go. Once it's gone that episode is gone. You can always catch EO again in the future. Also important to get a partner treated in case you are susceptible to a particular infection, so you don't get it back again.

      Hi Harvy,
      Thanks for the question.
      It all depends on the cause of the epididymo-orchitis. Usually I advise patients to take 2 weeks of doxycycline 100mg twice a day. And to wear briefs to support the testicles.
      still do a sexual health screen and you may have to do other investigations if the doxy doesn't help.
      Cipro is a good choice but if no improvement then does need to be reviewed in my opinion.

It was already 3weeks since i experience the pain and swelling in my testicle but after a week the swelling and pain is gone already but ive notice that there is something like a lump in my right testicle and ive been taking ciprofloxacin for 18 days, does EO is my condition and how long does this lump will be completely healed with continous medications?

      Sometimes you are left with discomfort due to inflammation after a course of antibiotics. Like a bruise which can take 5-6 weeks to heal.
      If gets worse then you will need to be reviewed.
      Remember the full sexual health screen.
      With regards to lumps on the testicles, you will then need an ultrasound screen.
      Don't hesitate to get a second review from another doctor if still having problems.

    I had Orchitis Epididymis 6 months ago. I had 2 week course of Oflaxicin which got rid of the infection. Months later l am suffering with severe epididymis spreading to the groin,thigh,scrotum and uretha. On my second dose of Nitrofurtoin which is keeping symptoms at bay but n ,cure. No uti,no sti detected. Now l find that my right testiclee is somtimes being pulled upwards when l walk. Painful and uncomfortable. Awaiting to see a urologist but it is all a mystery to me.

      Infection can cause a shortening of the spermatic cord, I'd advise a urological opinion as well, they may put you on a longer course of antibiotic.

      Very unlikely. E.coli is quite a fast growing organism, so if its in the wrong place then it will cause problems quite fast.
      If you are in pain, then get a review from an experienced medical practitioner.

    Sir, I was diagnosed with epididymorchitis through usg on 3/11/22. Due to extreme pain and high fever, I was forced to operate to clear the pus cell, which resulted in reducing my pain and fever by 12/11/22. I was stitched back on 17/11/22. However, even after 12 days of stich,heavy pus are leaking, though sutures in almost alternative day while dressing.
    After usg it is found that there is still acute epididymorchitis. The doctor is saying that infection will heal in time. As the fever is not getting high I have nothing to worry. But he is also telling that if pus not reducing in a month I might have to loose testicle.
    Sir I am in highly confused and worried. Please help.
    Currently takin doxy 100 twice daily but taken already for 13 days.

      Antibiotics is how you treat epididymo-orchitis.
      Why are you considering surgery?
      If the doctor has diagnosed another problem then this may require surgery.

      Have you had a full 2 weeks at least of antibiotics? If so which one?
      Also wear supportive underwear like briefs.
      Pain should start to resolve. Can take a couple of months.

      Yeap there are other antibiotics.
      Ideally you can catch and grow the bacteria.
      I've used doxycycline, co-trimoxazole, minocycline, moxifloxacin etc.
      But if an individual is sexually active Doxy usually tried first as its also an mild anti inflammatory 👍

    Hello Doctor
    I am 17 years old. Three months ago I had an orchidopexy to prevent a "likely to occur torsion". The surgery went okay. Though afterwards the stitches took a long time to dissolve or "fall" around 4 weeks and my doctor had to remove them. Everything was fine until around a week later. When suddenly pain occurred in my right testicle. The pain persisted and I returned to my doctor. He had me take an ultrasound and the report said it was probably epididymo-orchitis. I was given a course of norflaxcin for 30 days and I just finished it today. The pain and discomfort did decrease after the first week of taking the antibiotics but ever since it was been sort of flaring up. Painkillers do little to ease the pain. Currently I am confused. Were the antibiotics ineffective. Why hasn't the pain gone away. I am done with the course but the pain persists.
    I haven't engaged in any sexual activity of any kind.
    Forgive my english, it is a second language. Thank you

      Your English is perfect.
      Epididymo-orchitis can take months to settle down. Are you wearing supportive underwear?
      Think of the testicle as a golf ball on an elastic band. The testicle needs to be lifted to rest the elastic band... I hope that makes sense.
      Is there a particular point which is tender on a particular testicle or on the cord?

      @@DrBretPalmer No not exactly I did wear some when I was recovering from the orchidopexy*,
      When the pain flare ups occur I find wearing anything at all brings pain. My groin even hurts. The doctor said it was lymphedema or something like that.
      As for where the pain is generally located. It's on the lower and middle part of my right testicle. I don't know if that makes sense.
      When you say it takes months to settle down. Are you saying the pain will persist and will eventually go away. Because lately I have been at my wits end, really down because the pain seems to be always present. I have been thinking maybe the antibiotics were ineffective. I have been taking over the counter painkillers like cocodamol there and then when the pain becomes unbearable but have stopped today after reading it can cause liver damage and codeine causes addiction. I don't know what to do at this point

      And also I don't know if it's related but around 2 weeks after the surgery I started to feel a sort of pulsating discomfort feeling between my legs.
      Around 3 weeks later the pulsating or throbbing feeling became more intense. It can occur for anyway around 1--12 hrs then it goes away for a number of days. The throbbing not necessarily pain is enough to ruin the entire day. I don't know maybe it's the prostate responding to the infection or something. I did see a doctor but he settled at anal fissures. I did take take suppositories for that but the pain comes back now and then.

      It's a difficult one. You can go back to the surgeon to see if they can do anything.
      Can use doxycycline to see if that helps as it's also an anti inflammatory.
      If its definitely Epididymo-orchitis can use moxifloxacin 400mg od for 14 days.... stop if any tendon or muscle or joint pains. Ideally should be given under the guidance of your doctor.

      I do hard work most of time i take tight underwear and yes i saw urologist But what scares me is that is been more then 3 months now And its hard to deal with
      Also there is some swalowing that comes and go in lower part of testicule i fel the epididyme some time swollen and hard , big and when it relax its like hanging down and mutch bigger then the left epididyme tail

      @gm8004 okay, you say you use tight underwear. Shouldn't be tight, just supportive, to help lift the testicle. Pain that comes and goes should be more likely non infectious, and bacterial infections, tend to get worse. Get some comfortable briefs to see if that helps. What job do you do?

      I work in buildings so its a job that make you lift things ...so i had severe pain in right testicule with swallowing epididymo orchitis as diagnosis and it did some hidrocel i think for like 5 days before went to emergency. That hapened in march 18 till 23 march
      Doctor now 5 months later all look ok just some swalowing in epididymis and a lot of pain coud this be that the nervs are dameged do to inflammation ?
      I'm in so mutch pain im afraid of not beeing chronic😪

      @gm8004 good that looks okay. That's why important to rest and correctly support the testicle. If you are wearing too tight underwear or clothes, then you'll be crushing the testicle. Wear briefs and of an evening try no underwear, see how it goes.

    Hie Doc. I have swelling in the testicle which rotates to another testicle. There is a discomfort in the urethra.
    There is mild pain in the kidney area joined to the testicle.
    Painful and itching anus after going to the bathroom.
    Just a general discomfort and no actual pain.
    I have bought antibiotics over the counter twice after describing symptoms to a pharmacist nothing resolved.

      I would personally recommend that you see a doctor, ideally a urologist if there is rotation of your testicle.
      Ideally have a sexually health screen before you start any antibiotics.
      Anal itch, no soap just use water and emollient to wash but you should be reviewed by a doctor or experienced nurse in case you have a fungal infection.

      Hi there, the explanation is not quite clear. There is scrotal space between the top the the testicle if this it what you mean?
      The epididymis is attached to the testicle and joins the Vas Deferens in the spermatic cord going up to the prostate.

      Yes, in theory, it would be possible, but usually the space is very small for the sinus of the epididymis.

      Funiculitis is inflammation of the spermatic cord and needs investigating via doppler ultrasound if does not improve with antibiotics. Orchitis is inflammation of the testicle.
      Varicocele is enlargement of veins within the scrotum, usually not painful but can be tender.
      All three problems you need to wear supportive briefs. For the first two you need a full sexual health screen and review by a sexual health doctor or a urologist with an interest in sexual health.
      I would advise you to seek out a local sexual health clinic, for further investigation.

      May also need to get your prostate checked. If the Ciprofloxacin is working then have an extended course (4 weeks). If you've been on cipro for at least a week but no change then consider a swap to the antibiotics I suggested previously for 14days. Doxycycline or if that didn't work Moxifloxacin for 14 days.
      Have a discussion with your doctor. On Tuesday.

      Nope EO can be diagnosed on blood test.
      Full history and examination.
      If looks like a bag of worms in your scrotum (usually left) then varicocele.
      If testicle/s hurt then could be EO.

      @@DrBretPalmer yea it’s on the right side which abnormal to varicocele.. however has symptoms of varicocele, already had a blood test and nothing turned up in the blood test results. At first never felt it in testicles, but it moved from bladder region alongside base of penis then evolved to pain in testicles. Now it’s swollen veins on right testicle , hard to exercise and do things normally , can’t sit for long etc..

      You need to see a urologist if a varicocele is causing discomfort, that requires a surgical procedure. If you have no symptoms the can leave a varicocele.

I had an ultrasound and the symptoms weren’t there; however after the ultrasound i was wondering why the doctor was scanning there ; and then the symptoms / pain there came after.. so they wouldt have seen it during initial scan. However I had lots of discomfort well before that time without it showing in testicle it was in upper vas defernes right where it touches pelvis area and then moved down which is weird.

    I am 18 years old and have testicular pain, mainly in my left testicle for the last 6months, My doctor prescribed me with antibiotics to help with the dull ache/pain but it didn't work, I have had an ultrasound and the doctor found a small cyst on my epididyms but said it would just go away and nothing to be worried about. there is no lumps, no swelling, no discolour, no blood in urine, no problems urinating, only symptoms are a warm scrotum, and the dull ache/pain in the testicles? I am a virgin so cannot be sexual transmitted , Any idea what it could be?

      It may be due to clothes you wear?
      Do you wear very tight clothes e.g. skinny jeans which are tight around the crotch.
      Do you do sports which impact around this area e.g. biking or horse riding? Or impact sports like jogging or football anything that causes the scrotum to bounce.
      If none of the above I advise a good pair of supportive underwear, briefs, to lift the testicles and swimming as a sport for a month to see if this helps reduce the pain. Swimming also supports the testicles.
      But please don't wear ultra-tight clothes this is a common problem in young guys.

      @@DrBretPalmer Just to correct my age is 21 not sure why it type 18 but anyways, no I don't wear tight clothes such as skinny jeans I mainly wear trousers which are lose fitting. I don't do any sports at all, or any sort of movement/exercise that could cause the pain. Around 4 months ago my doctor advised me to wear briefs and I have worn them every day since, it slightly helps ease the pain during the day when I'm walking but only when standing and walking, sitting down the pain is just as prominent with or without briefs. The pain does seem to come from around the back of the tactical around the ephadims area which the sides of the briefs do actually push up against which can sometimes not too often can cause a slight pain. I do take very hot showers and the pain gets better after a shower so heat does seem to help it but only temporarily.
      I was diagnosed with type 1 diabetes 4-5 months ago around the exact time this pain started and its been here since. my doctor is going to refer me to a neurologist but know body seems to know what's going on and its been so long now that I worry if it is something serious its going along time undiagnosed.

      I think seeing a urologist is a good idea by your doctor, you will get a repeat USS and maybe a long course of antibiotics again.

      @@DrBretPalmer Yes my diabetes is very well controlled I am currently in my honeymoon phase which helps but my sugars are almost always between 5-7mmol
      Could the problem be something to do with the cyst? it definitely feels like its from around that area?
      Also are you able to rule out anything serious as its been a while and I haven't been diagnosed so starting to get worried

      I can't rule anything out without an examination and investigations.
      The testicles aren't growing which is good but you will need further review by a urologist.
      Well done with keeping your blood sugars under control, it can be difficult to achieve that and it will help alot.

      Viral infection can cause many symptoms so I could say never.
      However I am not aware of any cases I've seen personally but there are a few case reports on Pubmed website, so it does appear to be a rare consequence.

      Epididymo-orchitis can reduce fertility but only by a bit.
      Most men who have had Epididymo-orchitis go on to become a father.
      The biggest risks to fertility are smoking, over weight and bad diet.

      Could be left with a haematoma. I'd advise an ultrasound scan and a medical review if no change after the course of antibiotics.

    I m 17 year old male , sir I have pain in my right testes from the last 8 months. I first came when I was playing cricket and I was taking run and suddenly I noticed a pain in my right testes but I ignored at that time I continued playing after 20-25 minute of continuous running pain was increased. When I went to home I took some rest and pain goes away on it own. But one day I notices when I was doing some stretching exercises the pain comes again and suddenly goes away. Sir, whenever I run or do exercises pain comes and goes away on its own. Due to this I am unable to run properly. On resting or sitting or simply walking doesn't causes any pain but running and exercise do. There is no swelling, no pain while urinating,no redness in scrotum, but only pain and that goes too after taking rest. Please reply me

      Hi Walt,
      This is very unlikely to be an infection due to the nature of the pain.
      Do you wear supportive underwear? E.g. briefs? If not then wear them as your testicles need support.
      If you are sexual active then get tested anyway.
      Is there an area of point tenderness when pressed?
      You may also need to see a urologist. If in the UK this is via your family doctor. If else where in the world it may cost a great deal.
      Please try supports (briefs) first.
      Please let me know if you try them and what happens.

      @@DrBretPalmer sir I usually do not wear supportive underware but once in a match I wore supportive underware and literally I was running, pain was there but very less.But without tight underwear it hurts while running 😭. Sir I have never done sex . Sir, I remember once my testicles were hit by cricket ball one or two week before the incident but it doesn't hurt much at that time. I wasn't a hard hit .I think maybe this is the reason of my pain. I went to local doctor on Saturday and he gave me some medicine and told me to come after 5 days if still there will be pain then we have to do ultrasound.

      Okay, well you've answered your own question.
      When you are out and about wear supportive underwear, e.g. briefs.
      They don't need to be tight, you don't want to squash your testicles.
      Wearing briefs for 2-3 months should help your body recover.
      Don't have to wear briefs while sleeping.
      Still attend the USS, yo make sure all is good.

    I'm 21 and I've had pain that is really unbearing and it is frightening not knowing what I have. Ive had this problem for 3 years and I went to the ER for this 2 years ago. They did ultra sound, pee test and other stuff and told me I was perfectly healthy and Take Ibuprofen to reduce inflammation and it'll go away. Well it did go away for 3 months 2 years back, then it came back but very mild pain where it was bearable and didn't feel that painful. Now 1 month ago I've been in pain and discomfort and swelling came back and I went back to the ER only for them to tell me that I'm perfectly healthy and to make an appointment with my personal care Dr but I don't have insurance at the moment and now I'm trying to reapply for Medi - Cal. As of right now for the past 3 days I've been getting a leaking sensation from around my left testicle and it feels like liquid flowing down my left leg. It's a very uncomfortable feeling. I'm hoping I can see a Dr Soon. I'm not feeling as much pain in the left one as much like 5 days ago and only swelling and leaking. It does start to hurt sitting for a while so I usually switch between standing and laying down. Thanks for anyone who read all of this. It just feels good to vent my concerns on the internet.

      Lt testis and epididymis is bulky and heterogeneous in echo texture in its entire body and tail region and showing increased vascularity on color Doppler with markerd thickening of the left spermatid cord seen with maximum diameter and showing increased vascularity on colour doppler with reactionary scrotal odema seen with mild amount of reactionary is free fluid noted in left scrotal sac -finding s/o rt sided epididymo-orchitis with funiculitis.

      I'm not in US either. I'm assuming you are in pain? If so then you should be on a 2 week antibiotic course.coukd be doxycycline 100mg twice a day for 14 days or if very painful can consider moxifloxacin 400mg once a day for 14 days. Hydrocele not important yet as most you just leave alone.

    Hi Doctor
    I have EO since 3 months ago , took 1 month of antibiotic on the first month, left discomfort for 2 months until now and it didn't go away.
    Urine and blood tested - no bacteria
    Ultra sound report showing the result that mild EO after antibiotic course.
    (My doctor advice that no antibiotic is necessary and give probiotic and Rowatinex for the discomfort)
    Any suggestion from you?
    Usually the discomfort can take how long to heal completely?

      Hi Poi,
      It can take a long time for the pain to resolve, sometimes many months. I'd advise supportive underwear to help take the strain off the testicles.
      If a further sexual health screen including mycoplasma genitalium is all negative and USS was improving the rest and support is needed.
      If you exercise make sure it is low impact as well. Swimming is great if you are unable to swim then walking is also good.

      Sorry to hear you are still in pain.
      Have you seen a Doctor?
      Have you been checked for Chlamydia, gonorrhoea, syphilis, HIV and mycoplasma genitalium? May also need to be tested for TB as well. You may also need an Ultrasound scan of the testicles and then have the correct treatment.... usually 14 days of doxycycline 100mg, twice a day and a 1 gram of ceftriaxone given by a nurse as an intramuscular injection.
      Other treatments also available.
      Also remember to wear supportive underwear e.g. briefs to help lift and support the testicles.

      Hi there.
      Your English is great.... much better than my Danish for sure.
      Okay the treatment for epididymo-orchitis is 14 days of doxycycline, 100mg twice a day, if very severe and mycoplasma genitalium is considered then thus can be used 400mg once a day for 10 days. Must also use Testicular support (no boxers) and no heavy exercise if using moxifloxacin.
      Ureaplasma is a normal comensal and you should even be tested for it and it doesn't matter if it is present or not. In the UK we don't test for only in private clinics who want to generate a higher cash income.
      Do you have any other symptoms or just pain in the testicles?

      Im curently yousing a tight pair of underwear, but my Right testical still hurts and im taking a lot of painkilleres to take a bit of the pain away.
      My doctor gave me doxycycline for 2 days 100 mg but it did not help. Now he has given me Roximstad, i have decided not to do roxi anymore, beacause i feel weakness in my legs and i had a fever last night, my heart rate also increased.
      I have only had 2 sexual partners my entire life and ive bin in a relationship for 2 years, my girlfriend Got ureaplasma negative, i find it wierd beacause i havent slept with other womans after our dating started
      I dont feel pain other places in my body, than my testicle. Ive felt mentally down in a long time.

      And btw he gave me an injection of pain killer in my testicle and poked a vein so it has gotten almost purple for a week now

      I have limited knowledge of Roximstad as not used in the UK very much, we would use Azithromycin but after 14 days of doxycycline. 2 days of doxycycline is just pointless.
      Please don't use tight underwear as can bruise the testicle

    Hi doc, Im having pain in right testicle that comes and goes and discomfort in the lower right abdomen since a couple of months. I recall the pain began the past year. But i didnt have testicular pain back then...just pain in my lower right quadrant. It felt like appendicitis but no. I went to the gastroterologist. He told me to do blood and urine tests and he didnt noticed anything strange. He told me it could be just gas and to stop drinking coffee and alcohol. This was last year. But now, the testicle pain came in. Im kinda ashamed to go to the urologist even though i should. I recall having unprotected sex time before everything started.
    Does epididymitis starts with abdominal pain like in my case?
    Could it be an std even if the testicular pain began like a year after the pain in the lower abdomen?

      Lots of reasons for testicular pain. I'd advise to get a full STI screen, wear supportive underwear, stay away from tight jeans/clothes.
      If STI negative and other tests are negative (e.g. testicular TB if test needed) then a urologist may do an Ultrasound Scan.
      Don't leave it, start getting it sorted today.

      Same as in the video but may also have more wider symptoms e.g. Night sweats, weight loss, if in your lungs, can have a cough and produce a white sputum. Will need to get tested, not just a chest x-ray but also a blood test for igra quantiferon test.

    I had untreated epididymitis for maybe 9 years. It caused me to have low testosterone. I believe because of excessive masterbation over the nine years.. I'm still struggling to get back to "normal" 10 years after treatment...the treatment was antibiotics first then testosterone for a month and soak bath, no heavy lifting, no masturbation. Unfortunately I only took the testosterone and didn't do the rest....

      You need to support the testicles to allow them to rest.
      Full STI screen
      Doxycycline is very good for 2 weeks.
      If still very tender should have USS.
      If still infection can consider moxifloxacin under a doctor for 2 weeks.

      You may need a long course if not quite there.
      Can take 2-3 months to settle but doctor reviewing you is best placed to decide if you need more antibiotics, more support/rest or an ultrasound scan.

      Epididymo-orchitis is inflammatory and is painful.
      You should have another ultrasound scan, and do a pregnancy test to make sure you are not producing beta-HCG hormone.

    Followed instructions of 3 doctors

    But no rilif
    Second doctor
    levofloxacin 5 days but no change
    Both doctors given medicine for pain rilif also
    In short dosage gives rilif only during medication period
    Finally I got one general surgeon, he advised me scrotal sonography and
    That sonography doctor said funiculitis
    Doctor sir, proscribed me
    Doxycycline 100
    Defcort 12
    Multivitamin
    Evoin 400
    Systal fort
    Flozen AA
    After 14 days dosages I feel 70-80% rilif
    Then he prescribed (once a day)(for 1 months)
    Defcort 12
    Multivitamin
    Evoin 400
    Wear langot, scrotal support
    But now I am feeling I never visited any doctor same level of pain came back
    And also pain started in another testicle and lower abdomen
    Means now feeling pain both sides in inguinal area and testicle epdydmis and spermatic cord
    In very short
    Pain in testicle, spermatic cord, midpoint of inguinal ligament
    Perticular point pain at inguinal area 24*7
    Treated before one month as Funiculitis problem found in songrophy
    Around 80 % rilif that time
    Since 4-5 days pain came back
    Other side arises Same problem
    Now both side pain
    I can feel swelling in spermatic cord and back side of Right side testicle
    Please tell me exactly what I am suffering from

      Sorry to hear you are having a tough time.
      The usual length of course is 14 days of doxycycline. You have had a few courses so I can see why you was given levofloxacin.
      In the UK you would probably be given moxifloxacin for 14 days.
      Have you had a full sexual health screen?
      Need to be tested for:
      Chlamydia
      Gonorrhoea
      Mycoplasma genitalium
      HIV
      Syphilis
      If all clear your last doctor needs to review you ASAP, and send samples for investigation for possible infections

      Shouldn't be, most men have a testicle higher than the other. An USS would highlight any concerns but if its been like it for years then I wouldn't let it concern you.... if it does concern you then ask the doctor who is due to review you.

      If epididymo-orchitis is left untreated it can cause infertility, abcess, chronic pain can also occur

      Can be caused by non sexuall transmitted diseases such as TB.
      That's why it's important to see a doctor.

    Hello doc... I had EO last year and releived. Now before 3 days i had masturbation, and did ejaculate after 1 day.. Now slowly discomfort is seen in my left epidydmis... Im worried.. I took a blood test to confirm any infection. But every value, wbc and all seemed normal. Even now iam having some tension in those area. Should i take antibiotics or should i wait. Is it again a a case of EO?

      Wear supportive underwear and if eases then all good.
      If slowly gets worse then get a review by a doctor and can start doxycycline 100mg twice a day for two weeks, assuming no allergies. Keep skin away from direct sunlight and don't take with iron or zinc tablets.

      @@DrBretPalmer Doc, im having pain in pelvic floor region. Yes doc, supporting underwear helps a bit. But pelvic region sees burning🤐 and very difficult to sit now.

      The doxycycline helped previously? May need to get a review from your doctor. Also do Kegal exercises make it worse? (Watch the video on it, do a couple and you'll soon know if pelvic floor). If painful when you open your bowels (do a poo), then you will need to get your prostate checked. If painful or infected your urologist will put you on a long course of antibiotics.

      Yes sir... Im feeling tightness when doing kegel.. Very difficult now to sit... Have to c urologist.. Can u suggest any other exercise for this

    and am wondering why so hard to get antibiotic s. like what's the worst could happen . if they don't work I can stop taken them right.. make me feel like am asking for drugs . frustrating. iv got more advice from RUclips ( you ,) then a emergency room doctor in person . ridiculous
