Calling the mangaka a pedo for writing a deep story that occasionally involves the brutality of children is no different from people who called Miura low-life misogynist scum for depicting the ra*e of some women in Berserk (nevermind the fact that the first ra*e shown in the series was that of the male protagonist which is important for explaining his physical interactions with people later on. In Miura's case, the point was to exemplify pure evil and he did it well. He never glorified the rapists nor their actions. You're meant to hate them. Same for this show. It's hard to watch yes, but I appreciate artists who aren't afraid to show the absolute worst points of humanity in their works for the sake of making a point. The point being that there are many ways to portray absolute evil and there are even more ways to write a story. Some people are just bothered that the bad guys aren't typical shounen baddies which are only allowed to go so far. Long story short, it's beyond ignorant to judge someone's real life character based on how they choose to convey tragedy and evils in their work - as long as the actions are clearly frowned upon and not glorified in said work. End of story.
It isn't really for him portraying brutality, it's the needless sexualization of child characters. Not to mention he literally draws more nsfw stuff depicting similar things in his free time..
Look at one more ugly fact about the sequence from 2:18, that monster uses the hot iron to castrate her, you don't have to think too much to find out why, this matters at minute 13:46 when he realizes that that the girl is exactly like her, without a family and without the possibility of making one. The thing with the childrens will matter in the next episodes, I won't give spoilers but remember that everything in this anime has a meaning

The guy didn't castrate her, he burned her head, that's why she covers herself with the hat. You can see that in the manga
