So History, Review of Symptoms, the Physical exam, the assessment and plan as per what the doctor and the patient say. Attendings still have to do other paperwork such as administrative, billing, % disability, etc.

We can just send them to the eye doctor and wave as they later leave all happy. Thank you, Emergency Ophthalmologist!
Hey, ophthalmologist are important! After I was discharged after 10 days of hemorrhagic dengue, I had a weird side effect where the whites of my eyes swelled up! Thankfully, the ER had an ophthalmologist that night check on me to prescribe steroid eye drops. Swelling went down after a week.

This happened at my hospital in the first COVID wave. All routine surgery and non-essential clinics were cancelled so all the dermatologists and ophthalmologists were asked to support the critical care or acute medical teams. There was a great moment when one of the opthalmologists said that he could not possibly make the change as he could no longer recall how to cannulate a patient. One of the senior nurses offered to put on a remedial cannulation session for the opthalmologists!

I would do all the phone calls, set all the meetings, make sure everyone got their job done, and started a tradition which I dubbed "Scone Sunday", where each sunday I would bring in freshly baked scones, butter, and jam. People I worked with half-jokingly asked to adopt me on multiple occasions
As a former scribe, Ive gotta say, you nailed it. It was also interesting to see that only after a few months, all the doctors completely forgot how to use the EMR system because of us.

Almost every hospital and clinic has moved from using paper notes in files to an electronic system now, and the program can be a bit... difficult to handle at times for busy people. Think of it like a really really old looking windows 95 program that's a billion text entry boxes.
As a prior scribe, I love seeing how much the doctors need and appreciate Jonathon in these videos! It didn't feel like I was making much of a difference day to day, but man looking back I was at my doctors' beck and call for 8 hours straight
Scribes are amazing! I don’t know why more doctors don’t hire scribes. I’m an SLP so if I run in to my doc’s scribe I can always send a message for the scribe to tell the doc since docs can be hard to find sometimes. Plus, they learn the patients, they keep the notes accurate. It’s just fabulous!

As a critical care nurse who worked the covid floor through the whole thing, I appreciate your gratitude but grow weary of these nurses seeking their 15 seconds of fame by going on TV shows saying “I never signed up for this” and so on. Yes you did when you attached the letters RN behind your name. We work to help the sick no matter what’s going on. The hospitals have been slowly transformed into a “woke” places where gender identity and fighting RaCisM is top priority. I am worried about the future of healthcare when I grow old.

Thank you for making these. My dad is an opthalmic optometrist, but he started as a diagnostic team leader in the local hospital. He has a photographic memory and can still recite textbooks word for word and he’s almost 80. They called him back into work and my 61 year old retired ICU/CCU RN mother just last week. I show them your videos and they laugh so much while there needing a pick me up while on shift. So thank you for making this horrific time for our medical systems a little bit lighter.
One of the physicians who does IME's has a scribe and lets just say... the amount of stress that man carries is significantly less than the other doctors. Scribes are a well needed extra pair of hands and it would be great if it was a more available job! I feel it would help reduce the burnout soo much and reduce medical errors cause there is a second pair or eyes on it!

Also wanted to say that scribes do work hard! I remember writing 58 notes on an 8hr. H&P, transfer docs, etc. Obviously under the guidance of a physician.
This was me during my dietetic internship- there was a code blue while we were rounding, and the whole team started running. I didn't know what was going on, so I got lost and got there late, and when I got to the ICU, they had already resuscitated the patient. I wanted SO BADLY to yell "don't worry, the nutrition intern is finally here!" And I didn't even come with Jonathan the Ottoman. Hey, they were grateful for me when that same dude needed tube feed recs.
I worked through high school as a scribe and tech intern at an ophthalmologist office. The level of recognition and praise they gave to all of their support staff was amazing. They definitely know how much work you’re saving them and they definitely show their appreciation

My retinologist has a Jonathan. Or, rather, a Jonatha. The doctor and his entourage come in, he talks so fast while he’s examining me that I can’t even follow him, and then they all leave. Letters are already sent to my other doctors, prescriptions are already submitted, and a treatment plan is printed and handed to me before I can even get out of the exam chair. This video is 100% accurate.
As an Australian used to Government based health care I love your videos relating to US health insurance companies dictating which tests you can do and how doctors think they are in private practice but in reality are servants of the insurance companies. In Australia you aren't bankrupted if you don't have private insurance. You just have to wait longer for your elective surgery. If it's a life threatening emergency we're all the same, privately insured or not. Our private system is optional but it lets you get your elective surgery sooner.

I lost my two Jonathans when I scaled down my practice to work with one of my children who is a subspecialist who has never experienced having one. I’ve been an ED scribe since I started university and now I’ll be starting med school next fall. Even though the job is tedious and we don’t get paid much for it, I always enjoy being appreciated by the providers 🥰 Hopefully I’ll have my own Johnathan one day to appreciate too 😂

I've gone to a few appointments for myself where the doctor had a scribe. It truly saved time for the doctor. And, yes, the scribes were pretty quiet. Also, if you do some searching, there are certain times in history where scribes were used a ton.

During 2020 a lot of non emergency surgeries in my hospital (UK) were cancelled so many surgeons came wondering to our ward (gastro ward which ended up turning into a general one because of covid and lack of beds) looking to help out because they had nothing else to do.. me as a medical student had to walk an ENT surgeon on how to do a MOCA assessment even though the instructions were basically on the sheet, I'm still so confused by how nervous he was 😭
I was a scribe for an interventional cardiologist. The man would see 25-30 patients a day and took a month-long vacation every year. Catching up on documentation was rare.
As and ophthalmologist my self I can confirm this been 100 % true. In my hospital we where so short of staff that every single doctor have to do rounds at the ICU, gynecologists, trauma surgeons Everyone!! Things get very crazy for a while

Ophthalmologists can be so helpful in critical care. As someone who was struck by an icicle in the eye, ours showed up during the ED visit. He was such a sweet guy. Though a little out of place in the ED. I was lucky the ice hit the corner of the socket and not the eyeball. It was scary situation for a 6 year old, but he helped. He reassured me, tested my eyes, and gave my parents his personal number in case something changed.

I was amazed to see how scribes work with the physicians in one ER I nursed at. Good training for the pre-meds who scribed and their efforts made the docs more efficient seeing patients.
Having only the barest of understanding of what The Jonathan does, I would like to send him one gallon of . Soon all that will be felt of The Jonathan in action will be a quick breeze and sudden lack of paperwork.
the energy drink is a no no, the shakes and crash actually slow you down, just let them have one power nap for 10-15 min (really drives down levels of cortisol) and they will be good to go till they are done with the work. And they will clock out for lunch but is most likely still running around with a bar in hand.
I mean, you can do okay-ish with like one half of one lung. It'll be fine. Send him to the ICU. What corneal injuries on the sedated and paralyzed patients? I don't see any corneal injuries.
I am so appreciative to ophthalmologists right now. I had to go to an EYE ER in Philly when a regular ER in the Main Line sent me home for "eye strain." The EYE ER found optic neuritis and MS. They admitted to the Med- surgery wing of the hospital though because no other rooms were available.
It’s crazy cuz scribes and docs form this unspoken language. Like almost a mental connection after working together so long. When I was a scribe there were things I would anticipate for my doctor, even have a pen ready for signing 😂 . Most doctors would benefit from a Jonathan
Former Jonathan here, this is so accurate! Not only is Jonathan a scribe, but an office assistant, surgery assistant, ophthalmology technician, patient coordinator and random equipment repair man, amongst other things.
When I was a Jonathan in the ER. I'd literally be furiously typing for a majority of the shift and always felt like I was behind. I couldn't imagine what the doctors would have done without us there.

As a critical care nurse who worked the covid floor through the whole thing, I appreciate your gratitude but grow weary of these nurses seeking their 15 seconds of fame by going on TV shows saying “I never signed up for this” and so on. Yes you did when you attached the letters RN behind your name. We work to help the sick no matter what’s going on.
The hospitals have been slowly transformed into a “woke” places where gender identity and fighting RaCisM is top priority. I am worried about the future of healthcare when I grow old.

Thank you for making these. My dad is an opthalmic optometrist, but he started as a diagnostic team leader in the local hospital. He has a photographic memory and can still recite textbooks word for word and he’s almost 80. They called him back into work and my 61 year old retired ICU/CCU RN mother just last week. I show them your videos and they laugh so much while there needing a pick me up while on shift. So thank you for making this horrific time for our medical systems a little bit lighter.

One of the physicians who does IME's has a scribe and lets just say... the amount of stress that man carries is significantly less than the other doctors. Scribes are a well needed extra pair of hands and it would be great if it was a more available job! I feel it would help reduce the burnout soo much and reduce medical errors cause there is a second pair or eyes on it!

Also wanted to say that scribes do work hard! I remember writing 58 notes on an 8hr. H&P, transfer docs, etc. Obviously under the guidance of a physician.
This was me during my dietetic internship- there was a code blue while we were rounding, and the whole team started running. I didn't know what was going on, so I got lost and got there late, and when I got to the ICU, they had already resuscitated the patient. I wanted SO BADLY to yell "don't worry, the nutrition intern is finally here!" And I didn't even come with Jonathan the Ottoman. Hey, they were grateful for me when that same dude needed tube feed recs.
I worked through high school as a scribe and tech intern at an ophthalmologist office. The level of recognition and praise they gave to all of their support staff was amazing. They definitely know how much work you’re saving them and they definitely show their appreciation
As an Ophthalmic scribe myself, I do so much for my fellow technicians and the Ophthalmologist. I love what I do.

You've definitely started by now but always remember: The patient on the other end of the call is more afraid of you than you are of them!
My retinologist has a Jonathan. Or, rather, a Jonatha. The doctor and his entourage come in, he talks so fast while he’s examining me that I can’t even follow him, and then they all leave. Letters are already sent to my other doctors, prescriptions are already submitted, and a treatment plan is printed and handed to me before I can even get out of the exam chair. This video is 100% accurate.
As an Australian used to Government based health care I love your videos relating to US health insurance companies dictating which tests you can do and how doctors think they are in private practice but in reality are servants of the insurance companies. In Australia you aren't bankrupted if you don't have private insurance. You just have to wait longer for your elective surgery. If it's a life threatening emergency we're all the same, privately insured or not. Our private system is optional but it lets you get your elective surgery sooner.

I have been a practicing ophthalmologist for 40 years. Your videos are great and hilarious! I lost my two Jonathans when I scaled down my practice to work with one of my children who is a subspecialist who has never experienced having one.

I’ve been an ED scribe since I started university and now I’ll be starting med school next fall. Even though the job is tedious and we don’t get paid much for it, I always enjoy being appreciated by the providers 🥰 Hopefully I’ll have my own Johnathan one day to appreciate too 😂

I've gone to a few appointments for myself where the doctor had a scribe. It truly saved time for the doctor. And, yes, the scribes were pretty quiet. Also, if you do some searching, there are certain times in history where scribes were used a ton.

During 2020 a lot of non emergency surgeries in my hospital (UK) were cancelled so many surgeons came wondering to our ward (gastro ward which ended up turning into a general one because of covid and lack of beds) looking to help out because they had nothing else to do.. me as a medical student had to walk an ENT surgeon on how to do a MOCA assessment even though the instructions were basically on the sheet, I'm still so confused by how nervous he was 😭
I was a scribe for an interventional cardiologist. The man would see 25-30 patients a day and took a month-long vacation every year. Catching up on documentation was rare.

Ophthalmologists can be so helpful in critical care. As someone who was struck by an icicle in the eye, ours showed up during the ED visit. He was such a sweet guy. Though a little out of place in the ED. I was lucky the ice hit the corner of the socket and not the eyeball. It was scary situation for a 6 year old, but he helped. He reassured me, tested my eyes, and gave my parents his personal number in case something changed.
With the subspecialization of medicine, soon ophthalmologists will be running their own critical care units... it's gonna be called the "Eye-CU"

When I was a PGY1 doing a month in the ED, I saw a few Jonathans. Then I realized that in a teaching hospital, us residents were just Jonathans too :D

I was amazed to see how scribes work with the physicians in one ER I nursed at. Good training for the pre-meds who scribed and their efforts made the docs more efficient seeing patients.
Having only the barest of understanding of what The Jonathan does, I would like to send him one gallon of . Soon all that will be felt of The Jonathan in action will be a quick breeze and sudden lack of paperwork.
the energy drink is a no no, the shakes and crash actually slow you down, just let them have one power nap for 10-15 min (really drives down levels of cortisol) and they will be good to go till they are done with the work. And they will clock out for lunch but is most likely still running around with a bar in hand.
Be glad they didn't call in a psychologist, that would be a mess in more ways than one

I am so appreciative to ophthalmologists right now. I had to go to an EYE ER in Philly when a regular ER in the Main Line sent me home for "eye strain." The EYE ER found optic neuritis and MS. They admitted to the Med- surgery wing of the hospital though because no other rooms were available.

It’s crazy cuz scribes and docs form this unspoken language. Like almost a mental connection after working together so long. When I was a scribe there were things I would anticipate for my doctor, even have a pen ready for signing 😂 . Most doctors would benefit from a Jonathan

Former Jonathan here, this is so accurate! Not only is Jonathan a scribe, but an office assistant, surgery assistant, ophthalmology technician, patient coordinator and random equipment repair man, amongst other things.

As someone who went to the ER with a shard of metal in my eye, I'm glad there was an opthalmologist on hand.
When I was a Jonathan in the ER. I'd literally be furiously typing for a majority of the shift and always felt like I was behind. I couldn't imagine what the doctors would have done without us there.
