    I am 73 years old. I have started doing the exercises that you recommended and I really believe that they will help me avoid surgery. I had been seeing a physiotherapist for sciatica. This was a mistake as the exercises I was given aggravated my spine.
    I was recently diagnosed with lumbar stenosis. Thank you for your in depth explanation. Very much appreciated. My pains have subsided greatly.👍🏽🙏🏽

    I want to thank you for giving hope with the videos you’ve done about Lumbar Spinal stenosis…. Some other info online about it is pretty dark and depressing! I’ve had back problems that started suddenly 6 months ago (one minute it wasn’t there, then it was) and my physio thinks I may have lateral stenosis based on my symptoms and what hurts or gives relief when I move certain ways. It’s not a definite diagnosis yet (I’ve had no x-ray or mri) but I’ve been doing some of the core exercises you’ve given and also bending/stretching forward a lot for the last two weeks and it really does seem to have helped already.     Hello Dr. I love your videos . I have learned a lot .
    What do you think about spinal decompression with the Drx 9000 machine . I’m trying to avoid discectomy Surgery and I have already physical therapy ,meds and epidural injections and nothing has worked .

    awesome video, thank you for this video and information. The exercises and future therapy. I have had so many surgeries on my stomach that my muscles have ben cut and moved. I don't actually know all the the Dr's have done but have I trouble doing some kinds of therapy.

    Just a correction: there is no spinal cord below T12-L1 level. You keep mentioning "spinal cord" in relation to the lumbar vertebral column; but unless the pathology involves the conus medullaris at T12-L1 (or even higher), we really don't encounter the spinal cord in the lumbar spine.
    Next off, you forgot to mention foraminal stenosis (which may exist on its own, or co-occur with lateral recess stenosis and/or central canal stenosis). Foraminal stenosis that occurs on its own can be treated with lateral percutaneous/endoscopic approaches (either with a uniportal scope system or a biportal scope system) instead of the more well-used interlaminar approach.
    Lastly, there is no such thing as a "non-invasive" definitive treatment for lumbar neurologic compression (i.e., pinching of nerve roots, not of spinal cord). The definitive treatment of compression is decompression -- reclaim/create space around the lumbar nerve roots (not spinal cord) to restore blood flow and allow the roots to self-repair from reversible ischemic damage. Decompression can be done using open techniques (e.g., classic midline approach or the more modern & less invasive paraspinal Wiltse approach), can be done using more forgiving retraction hardware (e.g., tubular dilators combined with an operative microscope), or can be done using endoscopes (interlaminar & transforaminal approaches). We call the latter as "minimally invasive surgery" or MIS (which is the scientific term used by surgeons worldwide). Nobody says "non-invasive surgery" (because until somebody actually invents nanorobotic machines that can do surgery from the inside, that's still an oxymoron as far as today's technology goes).
    And yes, MIS for the spine (especially endoscopic surgery) actually isn't that big of a deal nowadays. Classic / open surgery of the spine is a big deal -- lots of muscle stripping & muscle damage, bigger bone removals & ligamental sacrifice, lots of blood loss, lots of pain meds after surgery, slow recovery after surgery, etc. Endoscopic surgery however is not at all like that -- patients are comfortable enough to be fully awake while surgery is being done, and patients can now be sent home as soon as the anesthesia wears off. Post-op pain management is easy with endoscopic spine decompression surgery too. Return to activity is rapid. Patients don't even require undergoing post-op physical therapy nowadays following endoscopic spine surgery.

    After extensions and back strengthening, I had an MRI and found to have congenitally short Pedicles resulting in spinal stenosis with ligamentum flavum hypertrophy in the L5. I have a aching legs and cramp easily. Would your series of exercises be helpful with this situation?

    I was told by my doctor that I had spinal stenosis when I was in my mid 30s I have had pain in my legs off and on from a very young age. I'm now 58 and have arthritis degenerative discs through my back from my lower back to mid back and neck I'm not completely immobile but very limited how long I can sit or stand fairly limited to what I can do. the original doctor said I was born with the stenosis is this possible or misdiagnosed?

    I have spondylithsis of L3-L4,L4-L5,L5-S1. I have been avoiding getting fusion surgery. I do have Rt leg sciatica pain some toes tingling . How can I find the specialist physical therapist to avoid this back surgery, in my area. I live in Orange County California? Can I reverse the spondylothsis?

      Hey Soumya, great question! It is usually a bit of both! The alignment of the bones in the back can start to press on the nerves that cause neurological symptoms such as pain, numbness, tingling, etc.

    ❤😢😢😢😢😢😢😢😢😢Please help!!!! I am constantly cramping up to the point that at one time my husband had to call an ambulance I had a full body cramp I couldn't even straighten up II was so stiff I wish and entire cramp I don't know what happened yes I do get dehydrated sometimes but it doesn't really have to do with that.       Start with one or two exercises at the lowest intensity. As you feel stronger, you can gradually increase the intensity. Consistency is key. Always avoid working through pain; soreness and mild discomfort are normal, but pain is not.
