Nutrition Care for Patients with Dementia During & After Hospitalization Webinar.

Patients living with cognitive impairment or dementia are often at nutrition risk. Hospitalization can exacerbate nutrition challenges, resulting in delayed recovery. This presentation will review key issues in and during transitions that should be considered to support these patients nutritionally.
