- Patients living with cognitive impairment or dementia are often at nutrition risk. Hospitalization can exacerbate nutrition challenges, resulting in delayed recovery. This presentation will review key issues in and during transitions that should be considered to support these patients nutritionally.
Learning objectives:
Consider the nutrition risk of patients with cognitive impairments and how hospitalization can exacerbate these risks.
Identify how the hospital eating environment can be improved to promote patient food intake.
Identify opportunities for transitions in care to the community to address nutrition risk and malnutrition.
