Hypersensitivity pneumonitis - fibrosis causes breathlessness? (+ other nuances)

  • A general overview of hypersensitivity pneumonitis (HP) based on the example from a comment received. We discuss the types of HP: acute, subacute and chronic, pulmonary fibrosis in hypersensitivity pneumonitis. It is also important to identify the triggers (antigen) for HP and this can be difficult. Sometimes exposure questionnaires may help, and we have blood tests to look for sensitization to birds (pigeon, budgie) or farmer's lung triggers.
    Breathlessness in HP is not always due to the pulmonary fibrosis alone so we need to look for other causes as well.
    Treatment for hypersensitivity pneumonitis aims to first remove the trigger (if identified and possible), followed by anti-inflammatory or immunosuppressant medication if indicated, or antifibrotic ("anti-scarring") treatment to slow down the worsening in the fibrosis if the disease is progressive.
    A very complex disease indeed!

    I am very grateful to have learned the imaging characteristics of hypersensitivity pneumonitis (right lung, airway, upper lung).

      not right lung specifically - predominantly it tends to affect both lungs but the upper zones

    Thanks so much. Yes, in my case these are pet birds we’ve had in our home. I’m 40 and don’t really have any other issues except for heartburn and spondylitis. I appreciate you taking my question.
    I’m getting a bronchial exam done this week. I’m hoping clear results come up.
    My wife and I are deciding to give the birds away; we don’t know for sure yet if that’s the cause, but it probably doesn’t help

    Thank you for making this video! I was wondering if you could tell me if you get bird fanciers lung because you didn’t clean their cage good enough. I’ve heard that but I don’t think it’s true.
    I was diagnosed with acute or subacute (I can’t remember which) HP in January. I’ve had pet birds for 20 years but started working from home last summer (so I’m around my birds all day now) shortly before my symptoms started. My CT scan didn’t show any serious scars. I tested sensitive for all birds on the HP avian test except chickens. My Dr said I could try rehoming some of my birds and taking mycophenolate. I rehomed 3 of my 6 birds and plan to keep 3 of them hopefully as long as the disease doesn’t progress. I understand I’ll always have it as long as I keep them but 1 of them is my ESA and absolutely must stay with me. So I plan to just manage having it and take lots of precautions. (Air purifiers, wearing masks cleaning, frequent cleaning, the cellcept) And my dr is going to do regular spirometry tests and occasional CT scans to keep an eye on it.

    Do you think prednisone would be ok during chemo for gastric adenocarcinoma? My father is 85y/o, and after his last chemo treatment, his breathing became significantly worse. He gets winded after walking for 5 minutes.

    Fibrotic NSIP .....can start immunosuppresant+antifibrolitc +steroid...three may prescribe?

    I have rotten moldy hay next to me 22 ft of my house blowing towards me lung problems because this morning I can't get it away from me and no one wants to believe that's what's causing all of it I said it happened in America I might be able to sue a farmer I don't want to have to sue a farmer I just wanted to go away and get it away from me I want with the cow poop
