      My son is incarcerated, having succumbed to obsessive thoughts (that I'll not detail here).
      I have severe ADHD and raised my children by the seat of my pants, struggling with addiction, broken relationships, and chaos. I've often blamed his unstable childhood for his OCD. Is it only neurological or is it complicated by childhood uncertainty?

      My daughter was diagnosed with severe OCD at the age of 15. Counting, checking, scrupulous and relentless hand-washing... she could no longer attend school or function. She received inpatient exposure response therapy for two months at a renowned adolescent OCD center. It saved her life. The next year, she graduated from HS with honors. Ten years later, she's married and runs a very successful business. OCD isn't something that will just go away entirely, but she feels in control of it and knows what to do if her thoughts start to go that way.

      This video was very triggering for me, because i have ocd since i can remember and the several professionals i have seen had done one of more of the types of therapy you mentioned.

      i was taught the military breathing technique box method and i think its worked, i find myself laughing while doing it and seemingly feeling better however my hand still feels heavy like its magnetised to what i want to touch

    I feel so called out by the light switch thing. Also, when I was a teenager I told a therapist that I was super anxious about leaving windows unlocked when I go and he said I should just check them as much as I feel like I need to because eventually I'll get tired and go to bed :/ Pretty sure that's how the compulsions started

    Hey, this is actually GREAT!
    I'm a psychology student and this is enlightening!
    Hearing an actual, technical criticism of ineffective therapy strategies like this is fantastic!

    I disagree with some of this. I've had OCD for 25 years and CBT/ERT therapy had only a minimal & temporary impact on me. Far more effective has been long-term psychotherapy, exploring childhood and attachment issues/trauma. It enabled me to 're-parent' and build a sense of trust within myself - something I had never developed growing up. As a result, I have much more perspective on the 'creeping doubt' that used to compel me to check things multiple times. Due to the deep therapeutic work, I feel much safer in the world and as a result, my OCD symptoms are massively reduced.

      Love to hear this!
      CBT and ERP are evidenced based practices for OCD… however, it doesn’t mean it works for everyone. It doesn’t necessarily get to the root because not all people are ready, able, or willing. It’s awesome you were able to dig in and do the work to address the roots. That’s no easy feat! 🩷
      Also, many people don’t know what you now know.. that OCD can be a result of adverse childhood experiences/ trauma.

      I agree. I did CBT/ERP treatment twice. Once as an 'outpatient', and once as an in-patient in an OCD treatment center. Once with meds, and once without. I did it full-time, both times, for a bit over 5 months (when I had to stop because I ran out of money, and I was physically, spiritually, and mentally exhausted).
      Neither time worked. I suppose I should amend that to--they both worked, but only to an extent. Both times I got to the same place in my hierarchy (very high) and I simply stopped 'improving'. I did ERP for weeks after the improvements stopped, but nothing changed. Years later, my then-therapist (NOT "working on" my OCD) said she believed my OCD was tied into C-PTSD. Well, THAT was interesting. I didn't even know I had C-PTSD. Since then, I've considered it and it does seem likely. I haven't yet had a chance to try therapy for the trauma. (Something about being poor and all.)
      But one thing that's VERY bad about CBT/ERP for OCD--the way doctors tell both patients and their loved ones that this treatment IS going to work. (And the name--there really isn't any "CBT" involved--other than to tell the patient that this is what's happening inside and this is how it's treated.)
      Now my family believes I'm still sick because...let's see if I can get this right...I'm LAZY, COWARDLY, and MANIPULATIVE. That's right! I have an illness that has led to my being on disability, unable to earn enough money to take care of myself, because...I'm a miserable piece of expletive. I CHOSE to stay sick. I CHOSE to hurt my parents. I CHOSE to have my life destroyed. And every day, those are the things I continue to CHOOSE.
      So, for all the OCD 'therapists' out there, I have some very choice words for you...but Google wouldn't let them show up anyway.
      To JJvisit - it's GREAT that you figured it out, you got the help you needed, and worked & fought your way to better health!

    Man I hated my therapist trying to dig through my childhood trauma to fix my ocd intrusive thoughts even made me felt as if I was psychotic, the online videos to stop ocd worked than the therapist that made me feel bad.

    I developed full blown OCD when I was 21, about 4 years ago(although it was definitely underlying and came out a few times before that which I didnt recognize at the time).
    I started out in psychoanalytical therapy and relate exactly to this video, I found the therapy good to vent my weeks log of anxiety but unhelpful long term. I self educated on different disorders and brought up OCD to my therapist, which he said he does not consider OCD as separate to anxiety, possibly due to the theories of Freud regarding anxiety or OCD. So I stopped that therapy as it was a waste as we were not on the same page. A year later I finally found a behaviour therapist that my health insurance would cover the costs of.
    The points you laid out regarding the right way of treating ocd were ultimately what made the difference for me. The top tier of my obsessions was the fear of developing schizophrenia, and the way I shifted the balance of uncertainty was by researching schizophrenia as exposure therapy, in measured increments at a time.
    But that wasn't the only thing, as cognitive therapy helped me be able to break things down properly using a similar method to the ABC but not identical. We also analyzed the thought circle of OCD and identified the points in the cycle we could intersect to prevent the loop from repeating.
    I still have other things that I obsess over but the belief of having schizophrenia rarely comes to my conscious mind these days, even though I may have the typical intrusive thoughts.
    I'm going to study a conversion course to psychology next year, and hope to get into a master's in Neuroscience after that, so I believe sometimes bad things happen to find true purpose.

      I had my "first" episode when I was 21 too.
      It took me 6 therapist before OCD was ever brought up, and I was the one to actually mention it first.
      My therapist gave me the link to the OCD foundation that has a lot of great book recommendations for specific types of OCD.
      But a big bulk of my turn around was with techniques taught by Katie D'Ath. She has a great RUclips channel. I felt like a different person after 20 videos. (they are numbered)
      I also bought her book on Amazon. Good luck everyone 🙏🏾. Find multiple techniques that work for.

    Glad I found this video! When my therapist told me the “why” behind my ocd wasn’t important, but rather what was more important was that we work on reducing the behaviors, prolonging the anxiety as much as I can, I was slightly saddened because I wanted to know the why, but she and you are right! I was wasting time ruminating on the why. The why doesn’t matter! Anyway, I’m two weeks into living with germs, it’s not as uncomfortable anymore!

    Just wanted to drop a big "thank you." I've been watching RUclips psychology videos out of curiosity and so many of them just feel bad therapists making 10,000 videos trying to validate people looking for excuses to validate their wanting to leave a narcissist in their life or whatever. I clicked your video thinking you were one of these people, actually - I've developed a bad habit of rolling my eyes at this stuff and rubbernecking the comments. You've obviously got a lot of compassion and you describe (very well, very accurately - despite these being generalized sketch comedy lol) several therapists I've personally had and why I avoided mental health 'experts' for years. Every SSRI under the sun did something to completely mess me up and they kept saying that I was rejecting getting better because I didn't jive with a single one as they failed to give me any specialized or meaningful therapy or better tools to understand my differences. I experienced a lot of difficulties in my life, as a result... And a lot of "normal" people who had watched or read a couple of things trying to pigeonhole me because they didn't understand how different my thinking is (likewise, I didn't understand it too well, but you eventually start to piece together abnormalities and how to behave "normally").
    Anyway my story goes on and on, don't want to type 10,000 words in here. Your video usefully describes much that went wrong with my treatment, and I hope people struggling with their own issues can use your advice to help them find better therapy and solutions.

    A great video. Love how you mentioned the ways of breaking a ritual in early states of a treatment and the mentioning of doubt as a massive factor. The latter is something I observe many colleagues tend to address not that much, even cbt's.

      ​@@chichi9851 Me too. I actually signed her up for an ocd newsletter using her email, but I dont think she's looked at it. Everytime I try to explaim what ocd is, I can't. I know all about it, I've even written a 4 page essay with 8 peer reviewed sources on it, but I still can't explain it out loud to someone. Maybe I'll try writing or drawing a comic. I can explain it just fine in my mind, but not out loud.

    First off, truly thank you for providing these videos on OCD - we are fortunate to have people like yourself willing to share their knowledge for free, to better inform and - ultimately - give people resources, education, and hope.
    So, I've been working in behavioral health most of my career, but I've never really interacted with OCD as primary before (I've worked alternately - and frequently co-occuringly - with SMI, SUDs, EDs, and inevitably, personality dx's).
    I was curious about your emphasis on OCD having a predominantly biological etiology, and how this affects the therapeutic alliance and collaboration. Has the biological underpinning been strongly substantiated... I know with mental health, it can be hard to get away from the bio-psycho-social model and find a possible causal agent.
    And given the various EBP's you cite, they tend to vary in scope of intent, which seems to reiterate a client focused approach. Which I have no doubt you are indicating... have you found certain modalities are more aligned with certain subtypes of OCD?
    Finally, do you think that the doubt/uncertainty is the fundamental underpinning of OCD? And if so, what are your thoughts on more mindfulness based practices?

    Hi Dr Ali, I am with you on most possibly all of this. I know it can be very difficult to help people with OCD, and in truth support groups , er, come with their own sets of dynamics.
    I followed and passed on ERP advise, and my situation has much improved.
    However, I never did do active exposures, and wish clear instructions were more readily available. Also there is the very obvious problem that if people can find a suitable therapist, they may not be able to afford therapy. I would be pleased to hear from you..

    Do you have a recommendation for a treatment modality for healthcare workers with contamination OCD? How do you manage the anxiety and obsessions when there is actual contamination?

    A friend with OCD often wants to talk about and analyze things we both remember from childhood, and since I know from my own experience that that can be helpful, I do. Sometimes I walk away from the convos with an uncomfortable sense that I haven't helped or maybe I just made things worse, but I have no idea how. Interesting that this video validates my gut feeling, thank you vm for sharing the info ✌️

    My partner has ocd and no desire for treatment nor do i want to push treatment on him. I have learned how to live with it peacefully and try understand how to avoid his triggers. The issue that impacts us is it causes him to waste a mass amount of water and paper towels. I mean upwards of a roll a day and running the water 45 mins at a time. (And it takes a lot of time out of us spending together while he does his cleaning rituals.) What can/should i do?..

    For me recently ocd has been taking control of my life and that my brain is telling me that all of the compulsions that I have ignored or not done in the right way. I need to spend a week finishing all of them until I have done them all. Then and only then I can enjoy my life and be happy. And when I try to do my rituals, either another thought comes in my head at the time I am ritualizing or I would have to repeat them or sometimes when I'm done with the half done with the pile of compulsions they would start to pile up again. Its really tiring

      Exactly the same as me. I describe it as a backlog of thought compulsions queuing up to be done before I feel I can go on. I can have about six situations to go through, lying awake half the night trying desperately to clear them. I may provisionally clear a couple of the worrying thoughts only to get stuck on the next one and then have to start again from the first one. Utterly exhausting. I write lists of them to go through in case I forget one or two and cross each one off when I feel that one has been cleared. Many times I’ve had to give up utterly exhausted and after a few days I recover and forget about them. Then it all starts over again, and again, and again. I’ve had over 70 years of this but I’m now able to muddle through each day and manage to ignore the thoughts. No-one understood or cared what I was going through so I wrote a book for my wife and kids to try and understand - no one could be bothered to read it. I published the book on Amazon in the hope it would help others. I called it: OCD - A Life in Torment

    I have OCD, I went to a therapist and I was taught basic breathing techniques and to "relax and give myself a break", but was never taught about how my brain works and why I get the thoughts that I do, absolute waste of time and money, you can act like a professional but an act is an act.

      And a licensed psychologist that I went to go talk to mainly focused on trying to get me to focus on eating healthy and getting enough sleep, this is a person who has zero idea how this mental disorder works.

    Honestly I have a ocd and I m suffering still with ocd and really don't know what to do ... I m sitting at home from 3 year and doing ntg because of ocd tried 4 docter already ntg working ... it fire me day and night .. I just have zero hope with life .. I probably gonna die because of this ..

      May I ask what type of medication? What are the side effects you are experiencing? If they are really bad thoughts id talk to your doctor asap.
