    Pneumonia maybe infection ongoing. Alt. Kidney problem. Blood tests. X-rays. Prob diabetes.

    As an Paramedic on-scene I would do baseline vitals, 12-lead and ask about any recent med changes and consider the Beta Blocker as an initial cause. The acidosis could be a reaction to the toxicity of the beta blocker.

    nerve conduction studies.Hb,Blood sugars.HbA1C,Renal panel,yes to Vit B12 level. HighK+ level could cause weakness and bradycardia.rhabdomyolysis would give tenderness of muscles clinically.....so i wouldnt keep rhabdo ' as cause of the hyper K+,which must be due to the CKD .....metformin is also a cause of low vit B 12......but @ 500mg BID its a low dose.....may be kept low in view of CKD stage 3....Ketones+ ...DKA....musings! Imagings i would request USG Kidneys ureters ,bladder.ECHO ,EKG...CSF would show ACD inGBS...any loss of fine touch/vibration pointing to posterior column ....i would think of peripheral neuropathy associated with DM2.,CKD...now added DKA
