      I would first classify it: Secondary or Primary Hyperkalemia by: 1. Taking targeted history, 2. Doing basic Lab work up including : Blood sugar level, renal function test ( BUN and Creatinine), extended serum electrolytes THEN immediately initiate Insulin therapy, and do further management.

      Basing on this Lecture, I would:
      1. Do Bed Side ECG
      2. Give Calcium Glucomate IV to protect the heart
      3. Start Insulin therapy ( taking precautions against hypoglycemia)
      4. Give Sodium bicarbonate, in case of severe Metabolic acidosis
      5. Arrange for Haemodialysis.
