I feel like maomao is actually enjoying her job at the palace and she is working with a concubine and her lady in waiting that would actually support her even though those 3 lady in waiting ate her snack
I think Emperors usually choose whoever has a boy to be the empress, though if there are mutiple he will choose the people he likes most which is why concubines will sometimes poison the person that is pregnant or even the baby after it was born

The Emperor hasnt chosen an official Empress yet, so all the ladies are still concubines at the present, although, of differing ranks.
