Pre-authorizations vary widely across the board for mental health and substance abuse, with most being payer specific. While most substance use facilities do see a greater need for pre-authorizations, that does not eliminate the requirement for mental health pre-authorizations as prior authorization is required for many mental health services, including but not limited to: Outpatient therapy and Peer Recovery Support. The best protocol for all clients would be to monitor all CPT codes for services rendered as pre-authorization requirements at the payer level are often updated and/or revised.
Co-pays are determined by the eligibility check and most patients are aware of an associated copay per their policy's guidelines. Co-pays are collected upfront, prior to services rendered. Patient Responsibility is an amount that is determined after a claim has processed and paid. If an amount is remaining after the claim is paid, that is considered patient responsibility. However, Medicaid policies and contracted providers will not bill out for patient responsibility due to contract limitation as an in network provider.

I believe she's saying "a DAP note" at that point. A DAP note is a 'Data, Assessment, and Plan' note. AZZLY uses a color coded circle icon in the auto-generate claims tab to let billers know when all notes have been finalized and/or signed so the billing team knows when claims may be submitted on a compliance level.
