When claim denied for dx code invalid or CPT code invalid we have to look for reason why invalid of this, whether it is diagnosis code inconsistent with procedure. Are there any LCD# or NCD# guideline which may affecting the relation of between both and we need to take it for this from insurance, also need to check other DOS paid or not, then only need to escalate to tier 2 coding team. In inclusive need to confirm for the procedure to which CPT being denied as inclusive procedure. BTW great video.

Thanks brother. This is honesty a usable information, however a little confusion between we can send a correct claim to the Medicare or not. and deductible amount send to payer or provider.

Medicare does not accept the Corrected claim , if the patient has Medicare and they've applied to the deductible you supposed to bill it to secondary insurance. Because sometimes secondary pays for Medicare deductibles and sometimes they don't in case or pt don't have secondary bill it to patient

If the claim gets denied as invalid DX or inclusive we can still probe the insurance and sent back the claim for reprocess if you are very good at probing.

26 modifier is for provider claim which CMS 1500
TC is for UB 04 which is for hospital claims

TC is for professional claims too.

Why we won't send corrected claim to Medicare. Correct me if I am wrong but we can send corrected claim to medicare with the corrected code 1 and for commercials 7.

If you want to check just try to send corrected claim to Medicare in any process, It will always picked up as fresh claim with 1 resubmission code

Medicare ko corrected (adjustment) claim jata hai. Maine khud bill kiya hai aur payment bhi aya hai.
Note: Don't know about cms-1500, i am talking about ub04 claims
