it's because of an artifact called "salmonsnake soul", it prevents the user from bleeding and losing any limb
defence stance also prevents limb loss for a couple of turns with the perfect guard buff

Get better equipment, you can get the miasma, sergal spear, necromancy, eastern sword and blue sin quite easily. (If you want advice I can help)
