I think there is a BUG with the scaling of starting weapon, especially if it's a weapon that does not meet the requirements. The Whirligig Saw should have a strength requirement of 18, so you shouldn't be able to pick it up at the start. Maybe that's why it's scaling so bad, like a weakened version of the original saw.
Lowering requirements shouldn't affect late game scaling. D scaling is D scaling it's just the level it scales off is lower at the start. I think the comment Zeus made was regarding the +0 weapon, low stats, and the absurd stats the enemies had.

I managed to get my Bloodborne platinum a few days ago, and I even got the old hunters trophies done. That was hell but with Orphan on it's last bit of health, and me near death I always remembered that one rule. When it's low, shoot that boss in the face. (Which I did to Micolash twice on NG+ and NG+2 just shot him from a balcony.) None of you blame me for that right? We can all agree he deserves to be shot in the face on his second phase from the balcony?
I never understand why people have such a problem with micolash. I find him to be as easy as Celestial Emissary, if not easier. The main argument I see is that a call from Beyond will one shot you, but if you stay next to him he won't use it. He only uses it if you're a certain distance away. And you can literally sidestep his other move, don't even have to dodge it. I guess he's one of those bosses that is easy to some players, but harder for others. Like how I couldn't beat blood-starved Beast by myself until my third playthrough, for some reason he's just insanely tough to me, also the only non DLC boss that I had help to kill.

I'm a huge fan of monster hunter and bloodborne, but yeah some fights in monster hunter can just drag on. The funny thing is mhw probably has the quickest monster fights in the series excluding the master rank quests. The time limits always seemed kinda dumb to me too. 50 minutes is usually enough, but imagine failing a quest not because the monster was hard, but because you just didn't deal enough damage in time.

You know the entire point of the long powerful combos in Monster Hunter is that you don't use them unless you know the enemy won't be able to move out of the way, right? It's all about learning the enemy attack patterns and using the tools available to you to make the enemy unable to move. For example in Monster Hunter World you can set traps that immobilize the monster you're chasing.

It would be very fun if they did get randomized, but I think they're just objects on the game world map that are made visible when you kill Rom/when you get enough Insight, they're not enemies. It may be possible to swap out the models used, but it would take additional coding to make it happen. Maybe you could send a request to the developer of the randomizer?
