How about people who have access to your calendar? (shared) - do they see the details?
As a general rule - no. If they are delegate for your calendar, then you can choose to let them see private appointments.
As a general rule - no. If you use the delegate option for sharing, there is a command to let them see the details of private appointments. Everyone else will only see message 'private appointment'.

Can you make is so that the people invited to the meeting will also see it as private?
Not that I know of. Private is a personal setting.
To the best of my knowledge, this isn't possible. Each person, after they have accepted, can make their appointment private.
does alex see the Private tag on his calendar?
Hi, this is an appointment, rather than a meeting so it's not being sent to Alex. However, if it was a meeting, then no, Alex wouldn't see the private tag. But he could add it into his calendar after accepting the meeting.

Under my categories, its just colors and no private :(
You're right categories has only colour options, but the Private command is in the same group - it should be near to it.
Which version of Outlook are you using? (Just to check)
@@michellekaye I believe it is the latest version. I just searched on the search bar, and it showed up. However there should be button where I can just click like you did.
I did explore more options under the my categories, but it just displays more colors.
