Legendary assassin and professional video gamer Travis Touchdown travels to Mars in pixel form to smoke space coffee/pot with a video game developer who murders him and then asks him to bring back peanut butter next time. What's so hard about that?
I'll give it a shot: the faxes Travis receives from K (Klark, Juvenile's romantic partner) reveal that Doctor Juvenile's Death Drive Mark 2 is connected to a CIA main machine called the Death Drive Triple-A, a device that 3D prints a clone of whoever has been connected to the Death Drive. Originally intended for setting up a base on Mars populated by clones of people on Earth, the CIA wants to use the technology for state funded terrorism, leading to Juvenile sabotaging the project. The final "game" is set in the real world where you control a clone of Travis that views the real world like it's a video game, meaning the bugs you kill are real employees of the CIA. The Hotline Miami arcade cabinets temporarily hijack clone Travis' vision and make the corpses look like the russian mobsters from that game. The final boss is Travis killing the real Dr. Juvenile and meeting the creator of the original Death Drive (either on the "real" Mars via being cloned again or in a digital version of it). Obviously, he's a bit annoyed and kills the clone Travis (this also fits with the theme of Travis having to stop playing games and return to his responsibilities, as indicated by Shinobu during the credits). Whether the real Travis is aware of what happened is up for debate, but considering the 4th wall breaking nature of the series (and that the clones can be remotely controlled), I wouldn't doubt it.
