I always thought it meant someone who was pretty on the outside hence the name madonna but were ugly on the inside hence black. Like they have a black heart. This a also sorta matches the lyrics

I think you take it too literally. It’s not just about color of skin, it’s more symbolic. Someone who appears pure but is secretive and shady. Like a double life, or duality of a person. It could be anyone really
