They are stuck there for a long time, eventually, the person realizes this isn't normal and notices that there aren't actual people or birds and the time never changes. Once they realize, they slowly change into their next life. It's too late they died. Or perhaps they were in a coma and in the end they died from it. Eyes shut tight likely means the denial of the situation. Wake up is the mind's attempt to make them go past denial.
