The meat contained in the blue cylinder is pressed forward into the casings (not shown) by the pink piston. The operator turns the orange crank to move the piston through the gear train and rack-and-pinion mechanism.
The interesting thing about this type of stuffing machine is that the pressure is created by the circular rack with annular teeth instead of the regular rack. It provides simplicity in manufacturing and assembly.
The video also demonstrates how to remove the cylinder from the stuffer.

We have similar at home, and still working! But the piston does not have on the shaft teeths all around, but just on bottom half of the shaft (that are pushed by the gear unit).

Does the rotational freedom of the pink piston help in getting a smooth fill? (I've only used an electric press, once, and didn't think to check whether the piston could rotate.)
The free rotation of the pink piston does not make much sense.
Here the circular rack with annular teeth helps to simplify the manufacturing and assembly.
