I just experienced this for the first time. I don't have 20K, but around 10K miles. I am a heavier rider, so, that may make a difference. I felt the same "0 confidence" in my bike as you stated.

Hardly ever use it. It's fine. Only replaced the outer ring.

Make you keep things clean, lubricated, and change out your chain before it is worn out (stretched).
you can not prevent it to not to happen but you can prolong teeth wear and from time to time reshape teeth on right shape
