    Sir,
    I've a patient with Kennedy class 1 upper arch,with intact lower arch she had tmj pain in the right joint and her lower posterior are supraerupted and contacts the upper eduntulous ridge,shall I open bite a bit to get room for rehabilitation of upper posteriors

    Hey Dr. Phelan! I was a member of Occlusion Design a few years ago - loved it! You helped me so much, thank you! Question: is alteration of the vertical dimension only possible when treating every tooth in the mouth? I had a wear case where I would've loved to lengthen the teeth more, but after Ortho I still didn't have a ton of room. All the posteriors were fine and didn't require crowns, so a FMR would've been unnecessary. You can see the case on my Instagram @dr.aaronpalmer . Anyway, thanks for always providing great info and being an inspiration. 👍🏻

      Hi Aaron, Great question. You do not have to treat every tooth in the mouth to open the vertical. If you watched the seminar linked above the first case opens the vertical dimension with onlays on the lower posteriors and no treatment on the upper posteriors. I have a case similar to what you are referring to with your ortho patient that we are starting soon. He has excessive anterior tooth wear but no posterior tooth wear so to manage him he is having orthodontics first to create room for the anterior restorations. I am planning to share this case with the members during a OD members webinar in the future. In your case ideally the orthodontist would have set the case up with the room for the anterior restorations when he was finished his phase of treatment. I hope this helps, all the best, Stephen

      a SAM 2 or SAM 3 articulator
