Very nice video. What´s your source for the definition of a pathologic perforator? Here in Brasil we mostly use Dr Labropoulos definition of 0.35s reflux time as the cut off
Is it spontaneous outflow or outflow on augmentation ?
If it’s on augmentation, is it proximal or distal augmentation?
What maneuvers do you do to search for reflux?
Sorry, I missed your reply/question. That is actually one of the issues. There are no completely validated maneuvers for testing for reflux in a perforating vein. In our practice, we typically do not spend too much time searching for perforators. Incompetent perforators largely are a reflection of venous system dysfunction. The offending superficial insufficiency will be addressed first, and we see how the patient does. If the ulcer or skin degradation is recalcitrant, then we look for and address perforating veins. In an overwhelming majority of patients, the perforator is due to deep system incompetence and in that case, you can make the person better by addressing it.
Beneath is proximal or distal or at the level of ulcer??? Please guide !
Sorry, I for the slow reply to your question. The consensus states under an active or healed ulcer but in our practice, we basically look for those in the vicinity and trace the associated varicosities or tributaries to the ulcer.
