    the layers of trauma there two absorb and heal together is what makes this story compelling imo

      First (major one we know of) was Riley, then Tess, then Henry & Sam, then Joel's injury, then David. All the shit they went through is ultimately all part of what the world became, due to the infected. It's what forms Ellie's personality and identity crisis with her immunity of being the potential cure for humanity. Ellie could be the answer to this, giving so many people the chance to maybe not experience all this savagery and suffering. It's pretty important to understand that for the last episode, as well as Part 2.

    This is the episode where we really fully MEET Joel, I think. We mostly see him through Ellie's eyes for the majority of the show, only getting occasional flashes of what Joel is/used to be, but this is the first time we get a real sense of why everyone who knows him is utterly terrified of him.
    The torture scene is CHILLING. Obviously because of how awful the actual mechanical acts are, but mostly because you can tell immediately that this isn't the first, or second, or probably tenth time that he's done this. He's practiced, casual, and efficient as he extracts information from and then butchers two men. He knows how to restrain someone with duct tape, he knows where to put the knife and how hard to twist it. He had the damned pipe handy because he knew he was going to beat a person to death with it before he even started talking.
    It wasn't the traumatic, life-altering thing that it would be if a normal person was forced to do this to someone to save their daughter. This was an unpleasant chore to him, like taking out the trash or typing up some TPS reports, and with about as much emotional impact.
    I think it could be argued that he was justified here given the situation, but god only knows who else he's had duct taped to that chair.
    I'm VERY curious to see what your take on the finale will be!

    Great to see you back with another reaction, hope you're doing well! This episode was especially close to the source material, which is why Ellie puts the penicillin in the wound, because she did so in the game (I agree with your reasoning that she's 14 and probably her best guess).
    This was the episode the show chose to submit for Bella's Emmy nomination and I can't argue with that, they fucking nailed every second of screen time. To me Bella's complete rage was cathartic, quickly changing to a depressing 'oh-shit' realization of the trauma and I feel I can see that on Ellie's face. And that 1000-yard stare at the end, absolutely haunting.
    Also, loved your discussion of perspective. Can't wait to discuss further.
    On to the fun facts:
    -The reunion with Joel was shot before the final confrontation with David, so the show runner organized a pre-shot setup where Bella was inside the restaurant with a stuntman who wrestled her around a bit and allowed her to play the hacking bit, before walking outside to film the bit with Joel.
    -When Ellie is 'hacking' David to bits, Bella said they used a chunk of foam filled with fake blood, so a lot of the blood spatter on the lens was practical.
    -Was shot on location (Waterton Lakes) in the middle of winter, Bella said the scene where Ellie finds David and James was the coldest day at -17C, so her voice slightly slurring is a real reaction.
    -Also in that scene, the rifle Ellie is holding is the real one, not a prop rifle. Show runner Craig though that it looked more realistic because Bella is so small and the rifle quite heavy that they were having a hard time holding it up, he was off-screen reminding Bella to keep lifting it up. I think it works.
    -All the snow is real, they trucked it in from outside the town for the shoot, then it snowed a bunch the night before shooting began.
    -When Ellie stabs James in the throat with the cleaver, they used a magnetic prosthetic piece in his neck so that the knife would hit that and stick to simulate going into the neck.
    -The actor who played James there was Troy Baker, who played Joel in the games, and he loved the idea of being killed by Ellie.
    -Bella has stated that the scariest part of the hacking of David wasn't the 'what have I done?" but rather the fact that a part of Ellie f enjoyed that violence, and that scares her more than anything else. Go back to episode 1 and check out Ellie's reaction when Joel beats the soldier to death.

    The guy who Joel killed did attack him and Ellie at that university, and was likely going to use him and Ellie as a food source, so I don't feel too bad for his family at all. Joel's violence was on full display in this episode, but at this point Ellie is a daughter to him and there is no way he wouldn't kill the entire village to get her back. In fact, in the game he does exactly that.

      Yep! I do agree. He can be a lot and still not be totally wrong 😊 he’s doing what he feels he has to (and in this post apocalyptic time, everyone is fighting to survive, so I definitely understand it!)
      But (spoiler alert) - the next episode’s rampage to save Ellie. Wow… I get why he was doing it, and totally understand that this was probably the only way, but it was still a lot and really intense…

      @@JessTheMD Yeah, Joel turned into John Wick. And honestly he was entirely in the right. The Fireflies were dreaming if they thought they could turn Ellie into a cure to save humanity. First off, saying she would've made the "right choice" if she knew was a lie because if they were sure of it they would've let her make the choice and not hidden it from her. They weren't honest with her that the cure wouldn't work anyway. The experts in the first 2 episodes who spent decades studying cordyceps knew there was no cure, but this doctor 20 years into a plague that eradicated all higher learning when he was probably an intern thinks he knows better and is willing to sacrifice a little girl on a bad guess. Not to mention they have zero infrastructure to mass produce a cure even if they found one. And no means of safely storing it for enormous trips around the world not to mention no means of globally distributing it anyway. This was a cure that would save a handful of people at most and it wouldn't have worked in any case. Joel made the right choice, period.

      @@JessTheMD In the game Joel gets tackled by one of David's thugs and falls from an indoor balcony in the university building onto the ground floor, on top of some rubble with rebar sticking out.       But if you want to experience it for yourself, it's definitely worth a play. Even the PC port is also in an okay state now, after a very rough launch. But it's also worth watching a silent 100% playthrough of it, due to the linear and very cinematic & dialog nature of the game. Just need to make sure it's Part 1 for the graphical and gameplay improvements. Part 1 is the PS5 / PC remake and uses the naming scheme of the sequel (Part 2), there's also the original without anything added to the title and the "remastered" version for the PS4, which is just the original with slightly better graphics. You might also would have to pause to read all the notes they pick up, if you're interested in the side stories / lore (one is for example about "Ish", in the sewers in the Henry & Sam part, which they referenced on a picture in the show). Feel free to upload recordings of either one if you want, especially if you jump into Part 2 afterwards. It's currently only for PS5 but I watched it even more on YT in form of a silent playthrough and then a lot of Let's Plays from various people. It's always interesting to see peoples reactions to certain moments. Plus you wouldn't really have to bother with any sort of copyright strikes in that case.
      I loved the show, but I still think the game is the better version, especially because there is way more dialogs & bonding between Joel & Ellie, especially if you don't rush things, because some optional dialog can be missed if you are not careful / rush too much ahead, especially when they talk, and some require to be in certain locations that might be a little off of the main route.

      ​@@JessTheMDIn the game Joel gets in a fight at that university and falls off the second floor onto a rusty piece of rebar sticking up. It impales him all the way through. The entry wound from his back and exit wound from his abdomen. It is a brutal injury.
      That kind of injury in the real world would be hard to survive, even if you could get to a hospital in a timely matter. They changed it for the show to be a broken end from a baseball bat.
      Another thing they changed from the game was that the infection was spread by spores in the air and you would have to wear a gas mask in parts of the game.
      A lot of things from the game are actually in the show though. Like, a lot. Shot for shot is the same and even some of the dialogue is the same.

    That was a very intense episode. Thank you for enduring it and sharing your medical expertise while doing so. I heard conflicting information regarding how the penicillin should be applied, a non-medical channel in there attempts to review information available to us online came to the conclusion that the particular form Ellie most likely had, based on the container, was designed to be administered to the muscles. I am glad you were able to share your experience of seeking to treat the sceptic infection with an IV and that the normal protocol would supplement the penicillin rather than expecting it to work by itself.

      Thank you! I didn’t look too closely at the bottle… I should go back and look! Penicillin can definitely be given IM (usually it’s the Penicillin G, and is often a treatment for syphilis!)

    One thing that cracks me up about certain shows and movies depicting post apocalypse settings is how sometimes they will have survival kits full of things like morphine, Valium, penicillin, etc.,
    But you can’t just to go Walgreens and ask for some morphine and Valium for a survival kit.
    They even require special prescription pads with multiple copies and anti counterfeit features and all that.

I felt your emotions at the end when he called ellie "baby girl" ( it's what he called his daughter) .. thank you for your medical opinions. It's great to hear a break down and explain each situation from a MD professional.     3:38 You know it must be a food pyramid because there's humans & human bodyparts inside it. ;)
    The pus during the second shot definitely was gone though, so I'd say the wound did look better overall. I think the cannibalism is one thing, but how we went about it another. Personally, if your group is having the choice of starving or eating those who are already dead, then that is definitely a valid choice to make. However, David's group attacked Joel & Ellie first, and the multiple bodies in the shed indicate that this was not just Alec hanging there. Now, there could be two other people that just died, or it could be two other victims that got murdered for food. David also did not give people the choice, but fed them unknowingly. He also gave the wife & daughter of Alec that weird look when they were eaten their husband / father, indicating that it was probably partially a sadistic power play from his side. Uruguayan Air Force Flight 571 is an interesting case on this topic. It's a flight that crashed in the Andes and the survivors were stuck there for two months. They only survived because they ended up eating the people that died during the crash or the consequences of it, like the cold temperatures or the wounds suffered from the crash. The wiki page is a good read, also from a medical standpoint, and I think there's multiple documentaries and I believe at least one movie about it too.
    Based on your wording I assume you realized that Ellie could only reach the cleaver because David was about to open his trousers with his left hand, lowering his grip on her arms. One change they did, compared to the game, is that they pretty much doubled the length of the cleaving and Joel, in the game, actually grabs her from behind, stopping her. I've seen many let's plays of both games and a lot of people end up literally cheering when she cleaves David, which I feel really misses the point of how terrible and PTSD inducing this moment is for Ellie. Doubling the length definitely seems to help to make it all the more uncomfortable to watch. Also, it was kinda stupid for Joel to grab her in that moment in the game, because that's a for sure way to get cleaved by accident as well. Additionally, the way Joel is during the torture scene, is a lot closer to how in game Joel was, even towards Ellie most of the time. He was definitely way more of an asshole.
    Definitely a tense episode, and an even tenser part in the game, because you switch from playing Joel to playing little Ellie, which gets hunted by a gazillion of men and then have to escape David, which is sort of a boss battle. Someone already mentioned that Ellie basically cleaved in game Joel here, so I just say there's one more important cameo coming up in the last episode, the most important one too. :)

    I'm glad you're back a bit more. I did have a laugh that the guy who was the second in command, who Ellie stabbed with the cleaver, was the actor who played Joel in the actual video game, Troy Baker. The actress who plays the game version of Ellie pops up later also.

    TW: SA (and sorry for the long post-I have a lot of feelings lol).
    Loved how we finally saw a glimpse of how dangerous and vicious Joel once was. It all comes out when he knows his little girl is missing…he violently killed Mrs. Alder’s mom in Ep 1 to save Sarah too. Moral of the story: Joel’s love language is protection, whatever that costs, and love can bring out the best and worst of us.
    My heart broke when Ellie killed David-you can tell she’s taking out all of her pent-up trauma, grief, anger and pain onto him. He preyed on her worst fear, which is ending up alone. So when she leaves the building, she probably thinks she’s alone until Joel shows up to remind her she’s not. I think this is the first time we’re reminded, as an audience, that at the end of the day, Ellie may be wise beyond her years, sassy and strong, but she’s still a little kid (14 but still), who’s gone through an unimaginable trauma of being violated and needs comfort in this moment.
    Bella’s portrayal of Ellie’s trauma response (the screaming, blank stare, panic, mumbling incoherently) was so raw and real. I think we could all use a big bear hug from Pedro Pascal when we’re sad/scared. 🐻❤️‍🩹-the way he delivered the line “Hey, look, it’s me.” was so gentle and soothing. Joel doesn’t save Ellie physically, but he saves her emotionally in that moment.
    I could be a little dark and twisty for saying this, but this is one of my comfort episodes. In rewatching it, I noticed that Joel grabs Ellie’s bag when he’s out in the blizzard.
    1) Such a dad move.
    2) He didn’t lose hope he’d find her.
    3) Holding on to physical representations of people he’s lost brings him comfort…Frank’s flannel, Bill’s coffee, he makes a rock memorial for Tess, and the watch/butterfly stickers on the wall in his room in Austin, TX/Boston, MA for Sarah. Had he been too late and Ellie was dead (he saw her bag, bodies and dead horse), he would have wanted it as it’d be the last thing he’d have of HER. 😢
    4) That bag has everything Ellie owns-it’s her whole life. Being a child who’s institutionalized as she was (in FEDRA school), and then abused can be so destabilizing, and her body probably didn’t feel like her own. Having her bag back was a way to bring her back to a sense of comfort/normalcy, having something that was still HERS. Shows you how considerate and caring Joel is. BRB crying again. 😢
    I’ve watched a lot of reactions from therapists and people who’ve experienced SA who say that the way Joel comforted her was such a great example of the way you should be with someone who’s been traumatized: he doesn’t ask her what happened (talking about it can be re-traumatizing), he just holds and rocks her, tells her she’s safe, and then bundles her up in his coat to help get her warm. Juxtaposed to one of the first things David (who views himself as a father to his group) does to Ellie when he takes her is strip her of her hat and coat-shows you what a true dad would do.
    I hope Pedro, Bella, Craig Mazin and Neil Druckman know how cathartic and healing that final scene was for many people, and I applaud them for handling such heavy, disturbing topics with such care. ❤

    It's very hard to find someone that will kill people. The world's army's have got this down to a fine skill, because people do not like killing people. So all these groups they keep finding that attack as soon as they see them, isn't normal without training or they all have lack of contchance. No emotional feelings. But it helps with the plot.

    10:44 By this point you’ve had a moral conundrum a couple times, but I’m steadily laughing, knowing what’s about to come. Joel was in no way wrong in this instance at all. Not only that, but these people attacked him and Elly at the college, unprovoked. So Joel hasn’t been wrong in his dealings with them the entire time. But more importantly, he’s doing that world a favor by taking them out. 😂

    I'm a little surprised that you didn't mention the paralysis danger Ellie faced when the horse was shot out from under her. My first thought in that scene was Christopher Reeve.

      You’re totally right! I didn’t think about it because she did move around pretty much immediately, which would not have happened with a spinal cord injury. Some spasms/jerks - yes; but not purposeful movement ❤️ but you are definitely right about that being a big concern with a fall from a horse like that

    I do like how you point out that Joel had done a lot of bad things. From an earlier episode, Tess telling Ellie that she and Joel weren't good people, and seeing Joel's absolute ruthlessness. It's a great set up for season two, no spoilers. Parr 2 is very controversial, no spoilers, but I fucking loved it and it's a great set up for part three, which will likely be season four of the show, since part 2 is being done in two seasons.

      @JessTheMD I have a measure of admiration for those who reacted to his ruthlessness as you have. People like you have an understanding of his ruthlessness and an aversion to his actions. I admire that. Perhaps there's a twinge of envy in me. I lived through some dark times and it drove me mad, I did what I thought I needed to do to survive and hurt a lot of people. I'm not living that way anymore, am striving to help others and alleviate suffering wherever I can, however I can, but I do it with the ruthlessness that I used to survive. That troubles me.
      I've known people who have survived r*pe, torture, attempted murders, and countless depravities, and at no point did they let their hearts become cruel, at no point did they desire to hurt people. They succeeded where I had failed for so long. I admire people like that and strive to be as they are, kind in heart. They showed tremendous courage in holding close their desire to love and be loved, they never let the cruelties heaped upon them, consume and replace their values. They sacrificed themselves, rather than allow others to be harmed, and managed to hide from their abusers the most beautiful parts of themselves, their capacity to love.

Thank you for your kind words, and loving the content. If you get the chance and inkling, Bates Motel would be a great series to review, to watch at least. Vera Farmiga plays Norma, and she knocked it out of the park. Great cast all around. Only four seasons and each end with jaw droppers. Or is it five seasons? I just know that I haven't seen the final season. Freddie Highmore is amazing as Norman Bates.

    He was stabbed 2 episodes ago, still the same day she drug him to the house and had the flashback the same day. This episode starts likely the next day from the stabbing.

    You have probably already watched episode 9 and have to edit the video before uploading it. I'll avoid spoiling it in case you haven't yet though.
    Let's just say episode 9 has A LOT for you to comment about from a medical perspective as a doctor.
    Especially how they just jump to the most drastic option as the first and only option. That's not how medicine works. Lol. It makes for great narrative though.
    After you finish episode 9 I recommend you watch as few RUclips videos showing you the major scenes from the game and from the show. There are a few of those videos on RUclips that are fairly short.
    So the guy that got a meat cleaver in the neck in this episode was actually the actor that played Joel in the game. Both the voice and the motion capture.
    In episode 9 the actress that played Elle in the game is in the episode. Can you guess who?
    Also the actress that plays Marlene in the show also played Marlene in the game. The same exact actress. That rarely every happens.
    So there will be a season 2 of the show. They said they will most likely break up the 2nd game into a season 2 and 3, but now with the writers and actors strike who knows when that will actually happen.

      I did know about the meat cleaver guy, but I had no idea about Marlene and Ellie!! My guess is the actress that played Ellie in the game played Ellie’s mom in ep 9? 😊
      I hope this strike ends soon (and favorably for the writers and actors! 😊)

      @@JessTheMD You guessed right. The actress that played her mom in episode 9 played Ellie in the game. That whole scene (with Ellie's mom giving birth) was not in the game at all.
      The way episode 9 ended abruptly like that with Joel lying to her was how it was in the game. One main difference that I remember from the game was that they each had a horse at that time instead of a car in the show.

    Great reaction again Jess! I thought the same thing when Ellie injected the penicillin directly in the wound, but what else could she really do? Apparently enough of it made an impact (at least for the purposes of the TV show). Only one more to go. I've really enjoyed this series from you (it's what got me to sub). What are you going to do next?

      Thanks so much!! Yeah, I guess technically she could have done an IM (intramuscular) injection (ie in a large muscle group like the glutes), but realistically, if he’s that sick/septic, IV is the preferred route. If this is Penicillin G, it can be administered either IV or IM and I would prefer IV given how ill he is! 😊
      In terms of what’s next… let me know if you have any specific ideas!! I have various random episodes of shows on my list that people have asked for (scrubs, house, greys anatomy, etc), but that’s it so far 😊

    I watched another reaction from another MD who said that penicillin should be injected into a large muscle group. Does whether it’s intravenous depend on the type of penicillin? If that’s even a thing? Does where Ellie injected to any good at all?

      Penicillin *can* be given IV (intravenously) or IM (intramuscularly - injected into a large muscle group). With how sick Joel was, I would have preferred the IV route!
      Definitely there are different types of penicillin. For example, Penicillin G can be given either IV or IM. I would expect them to have that form of penicillin, and so, one could argue that it should have been IV.
      Just injected in the wound… it probably does minimal good, if any. There *might* be some systemic absorption, I honestly don’t know… but I can say with certainty that IV or IM would have been more helpful 😊

    Right, Ellie is 14. Unless she took EMT training at Fedra, she would not know how to find a vein much less accurately put medication in the vein.

      Ha, those people might potentially be SOL in the apocalypse! 😝 I guess we didn't even know (initially) if Joel was allergic to penicillin. Good thing he wasn't! Are you asking specifically what we might use instead of a penicillin based antibiotic for an intra-abdominal infection like this? We could use cephalosporins (like cefepime), fluoroquinolones (like ciprofloxacin or levofloxacin) and flagyl (metronidazole). That is a potential alternative to something like Zosyn (pip-tazo) 😊

    The antibiotics/penicillin for Joel's infection is for intramuscular injection only, so Ellie is not wasting it, and is right not to administer it into a vein, though it preferable should have been in the buttock. I'm a bit shocked that you do not know this being an MD.

      Penicillin G can be given EITHER IM or IV. And if he is septic, he needs IV antibiotics. IM will not cut it.

Listen, there is a reason why it's for intramuscular injection only and I personally also received antibiotics + penicillin to fight a serious infection like this myself with great success, so apparently I'm lucky to live in Scandinavia, having had access to being treated by the professor also treating the Queen of Denmark. They obviously used a lot of money and research making this series. Spoilers below :
      According to season 2, a recorded audio log made by a surgeon reveal that Ellie is not immune, but imply that she must be infected by a strain of cordyceps that is used to make cyclosporine that is an anti-fungal penicillin that keep her safe from other cordyceps infections. But we will see how they adapt this from the game to the TV series, or if they will change it.
