Is it just the peacemaker or is it other weapons? Try changing your load order around with tactical reload. Or maybe you may have missed a patch or something.

Most likely a mod from a modding discord. Try looking on the modding servers you'll probably find it. It's either that or maybe its private but unlikely.
The sjörgen inertia is one of the best mods out there imo. It comes with a small quest, and (basically) two special versions, where a defective one turns into a melee weapon after a while. It’s also super satisfying to use 🤌🏼
