Definitely the supervisors fault. He's responsible for his workers safety. Seems he didn't really care about the plant washer or his concerns about a strong smell. Gave him a "paper mask" which does nothing for odors/gases and failed to investigate the smell any further. Guy lost his life because a crappy supervisor that didn't care enough.
Working instruction is not rigid and good good enough to work safely, especially with hydrogen. HAZOP Study should be reviewed by expert before next product batch.
These guys missed the first step in safety everybody who worked around that substance and in that mixing area should know what those substances are should know what off gases they produce should know how risky this stuff is they're dealing with.
The process of making food chemicals can contain some toxic reactants, that doesn't mean that the final food product contains the toxic material. Like the famous example, Sodium + Chlorine = Sodium Chloride. Sodium & Chlorine are both deadly, but Sodium Chloride, the desire product, is just table salt.

So, a low emotional state would be you working around someone that is in constant apathy for example with an attitude that there is "nothing I can do about it." Try and work around truly high emotional state people or don't. It's your life anyway, do what u want. Good luck.
