If you work for an agency, you’ve already agreed to your hourly pay. You perform duties for the client. The client is contracted with the agency and not you. Thus the client is not paying you.

It varies, you have to know your numbers ie overhead, employer taxes, going pay in your local market, rates in your local market etc. Based on national surveys the average profit is 30%.
Just as an example, the client transitioned to consumer directed. Care giver quit, the agency got hired by the client under consumer Directed. The consumer directed client offered the same job Same hours but double the pay. How do we justify taking that fifty percent that could go to the caregiver?

It depends on your state's rules & regulations. If you are in a non-regulated state then there are no specific rules. I recommend following your agency's policies. Create a job description and identify the credentials you would want that position to have.
You don’t count hours...you count days. However, the caregiver pay is pretty involved. Personally, I don’t recommend Live-In care. I have a video here on RUclips explaining the Live In process. Take care

how do we as caregivers know how much we are suppose to get paid from a client as the caregiver not what the agency wants to pay us?

For va billing is it one hour per unit ?? Or 4 units per one hour??

1 client is working 168 hours a week/ 24 hours a day. Is this for a live in caregiver? Math never been my strong suite.
Not not a live in. Two (12) hour shifts. Ex: 24 hours a day X 7 days a week = 168 hours X your fee charged to the client.
