Much like the Steel Knuckles, the Steel Spear gets most of its value from the its Perk Book series. With the books, the spear applies bleeds and heavy attacks pierce through multiple targets(useful in choke points). With the full set of books, kills with the heavy attack also completely refill stamina. I am pretty sure the Spear skill also makes it penetrate armor. I still prefer other weapons, but those perks can make the spear useful.
