When judging the people from the 1800 we have to be careful not to do so from the lens of today. The world, and life in general, was much different than it was today. It is easy to think of them as wicked compared to today's standards.
The medical kit at the end is a brace and bit drill with several drill/burr bits along with other tools. It's probably for use on bone, perhaps even the skull.
The .30-06 ammo shown with the Japanese knee mortar round is an En-bloc clip from the M-1 Garand rifle. If I'm not mistaken, the black tip indicates AP (armor piercing) rounds.
Definitely trepanation drill set, google search gives a lot more similar kind of antique tools.
Knee mortar round still has the safety pin/clip inserted- still a very dangerous piece of explosive ordinance. Get rid of it most immediately!!
Definitely a trepanation set. On the left, a hand brace and bits. The two "keys' are for making small holes. On the right, two periosteal elevators, the knob for the brace. three items I cannot identify, and a scalpel.
It might be a set of kauterions. Burning (cauterising) irons for stopping bleeding and to prevent infection. Or burn out infection.

There is/was no 'knee mortar'. It was meant to be held down against a log when fired vs tote a heavy tripod-type setup to use it. GIs broke a few legs when trying to use them from a knee.
that medical kid looks like it has skull trapanning bits and hand crank, as well as cauterising irons. I see the scalpel there, but I think most of the other stuff is for skull work. The hooks probably to hold veins and arteries aside.
