    getting a doc to first, Rx a CIMT and THEN actually finding a facility that performs them may be the bane of my existence.

      My cardiologist me do the Doppler. It was a COMPLETE waste of time and money. My results were clean on one side and "less than 50%" blockage on the other side. My Virta doctor said, "What good is that? Is it 2% or 40%" My cardiologist just said, you need a statin.

    I had tonsil and tongue cancer 2 years ago and had to have radiation to my throat and tongue area. I survived the cancer but couldn’t swallow for 20 months, but finally got off the feeding tube. Last time I saw my Dr of Radiology he said we now need to check my carotid artery to see if it has been damaged. Wish they had discussed all this before treatment, I have no idea what I’m looking at now but I’m praying to God it hasn’t been damaged. Gotta love radiation, it’s the gift that keeps on giving. I think I’m going to ask them to do this test. Thanks for the info Doc.

      @@tomdehen it was rough Tom, but I had my “WHY’S” as to why I fought like the 3rd monkey getting on Noah’s Ark. I will admit there were times during radiation, chemo and not being able to eat by mouth for 20 months that I wished I hadn’t lived thru the surgery. But GOD, MY FAMILY & FRIENDS rallied around me and got me through the nightmare. I wouldn’t wish a head and neck cancer on anyone, it’s a BRUTAL JOURNEY to SURVIVE IT.

      @@nettewilson5926 yeah well that’s why I took 300 mg of melatonin before each radiation treatment, yes I said 300 mg 2 hrs before each zap! It’s suppose to help protect your DNA and help the chemo and radiation work better. I ask my Dr at UTSW in Dallas could the body heal from chemo and radiation and he said “Yes, but it takes 7-10 years”. So I’m going to stay positive and keep BEING THE LOVE AND THE LIGHT in this dark world until my MISSIONS are completed down here and I’m called back home.

      Started with a 5 day hot water fast. Zero carbs zero sugar. Drink hot water and intermittent fasting 16 hours-20 not eating then eat mostly meat, eggs, cheese lettuce and mixed salad. No dressing. Usually do a 2 day fast when I hit a resistance area. Stopped statin, still on one blood pressure pill. 81 mg aspirin daily.
      Drink Zevia as a treat when eating.

    I recently had the CIMT and my arteries were clear/normal, I also had the Calcium Score Test weeks later with a score of 529, my Cholesterol and Triglycerides are all good, I am also a Type 2 Diabetic and have been on the Keto diet for the past 4 years and lift weights and walk 10,000 steps per day.

    What a wonderful teaching!!! Thank you so much! I have been working hard at keto/carnivore diet for a few months, and I really appreciate what you suggest about the CIMT test. I have no insurance and am on a fixed income, but I think I can save up and have this test done in the next month. Also, my Mom and Grandma both had strokes, so I am pretty concerned about my arteries, especially because I have strange pains and pressure in my neck. So thank you!!!! I will return and watch this after I get the test done.

      @@PrevMedHealth I'm scheduled to get the test next Thursday. I am forwarding this video to Direct Imaging LLC so that they'll know what I want. Humana Medicare does pay for it if ordered by a PCP with the right diagnostic and billing codes. With my plan, it will cost me $50 copay.

    16:03 min mark: if there is plaque from the CIMT exam, there's no need to panic. It's all fixable. 100% of atherosclerotic events are fixable.

      @@versewriter8123 19:57 min mark she says the fixes are (1) diet and exercise--she stopped eating high carb items like pears, (2) taking a statin, but she recognizes that many people have issues with statins--Dr. Brewer notes at the 1:05:00 min mark that Livalo (Pitavastatin) has fewer side effects than most statins, and (3) surgical intervention for those with very bad plaque, i.e., 70% artery blocked.

    I had surgery this year of my Carotid Gland, because they found a little mass, and they have to tested. What happened was they couldn’t go ahead with the biopsy, because of excess of calcification, thanks God…it was benign, but I am dealing with it!!! I’m a female 73 years old…

    I'm 59 yo and I've been on Carnivore for a year now. I work out daily, walk and ride my dirtbike 2 times a week. My only vice is George Killian's beer at 15g of carbs per 12 oz bottle. I know they are bad for me, but they are damn tasty.

    Is the CAC (coronary artery calcium) score test basically the same as CIMT? OK just googled it and ...."A CAC scan is better than a CIMT scan for predicting cardiovascular disease events. However, a CIMT scan may perform better in younger populations where CAC is less common."... So basically similar but done differently.

      Calcium score only tells plaque and does not break it down to hard or soft plaque Dr Brewer has many videos on this

    Dr Brewer, PLEASE give us the CPT code for this test-- it is very difficult to get med office folks to distinguish between this and doppler test.

    She also says you can reduce the inflammation with diet change and exercise, but what if you have inflammation in the arteries due to auto immune disease such as rheumatoid arthritis? What can you do about that type of chronic inflammation and the impact it has on the heart?

    So if I have been told (a few days ago) that I have a total CAT scan showing Calcium score of 118 I assume the CIMT would be a waste to do it. One of the arteries is 80 - one is 38 and three are 0"s. I wish I could hear from you.

      Yes, a CIMT screening is only performed to estimate your risk of coronary artery disease. Since you already know you have plaque in the coronary arteries, the screening would be a waste of time and money. That being said, if you and/or your physician want to know if you are also developing carotid artery plaque (for stroke prevention), a carotid Doppler would be worthwhile and covered by insurance due to your known coronary artery disease.

      Been following Dr. Brewer for years and he points out that CIMT has no ionizing radiation, unlike CAC's. So CIMTs can be repeated (to monitor results of your lifestyle improvements) and are low cost if you elect to get one without a doctor.
      My 2019 CIMT showed low risk; then a CAC score of 0.51 in 2023 seemed OK; but a 2024 CIMT found a "1.5H in bulb" plaque on one side. Now that I have plaque at age 70, I plan on another one or two annual CIMTs to monitor progression of plaque, since aging typically drives rate of plaque progression and since I have been avoiding statins.

    So the how do you go about stabilizing the soft plaque. I know about the diet aspect but she mentions statins can stabilize plaque. Is this true, because so many things I have heard lately say that statins don’t really reduce the risk of cardiac events?

      They don’t but they do cause a gambit of other terrible problems. Dementia, extreme muscle deterioration and your heart is a muscle. I use 1/8 of a tsp cayenne pepper in a little milk to keep optimum flow in my arteries.

    I want to access it. I saw a life line screening add it said, "need a CMIT" . It was a four part screen but all they measured was blood flow. Not the thickness. I'm in Olympia wa . Where do I get one.

    I am an advocate of CIMT because it is a better option than everything else in that medical device space, but to say it predicts 98% of heart events is just misleading. The test has a high sensitivity only because it has a poor specificity. The pie chart at 14:14 demonstrates this, 85% of the people she has tested score as elevated risk, only a tiny fraction of those with an elevated risk are going to have a cardiac event in the next 10 years. Using her math a test which categorized everyone as elevated risk would have had a 100% predictive accuracy. eta: I suspect the manufacturers of the CIMT software arbitrarily categorize so many people as elevated risk because such a diagnosis drives repeated testing.

      Aspirin, blood thinners, you can ad a low dose statin for its antinflammitory properties. Years working in the industry, I did a B-mode carotid ultra sound and saw a shadow which meant a plaque. I have been taking aspirin ever since, 30 years

    D only thing that may not be accurated is that those test dont see they inner carotide it is a Y shape the second part is in the head part?..

      Unfortunately, I have the same thing family history and heart problems w soft blockages, I'm only 41, thought I would be able to avoid it since I was always healthy and eat healthy, and exercise 6 days a week, walk every single day too at least 10000 steps. It was disappointing to say the least when my angiogram wasn't all clear. Far from it, and that they wanted me on a statin and beta blocker. Sometimes if it's familial we can't avoid it completely, I'm sure if I was not following a healthy lifestyle I might of already had a heart attack. Doing my best, at the end of the day that's all we can do!!

      ⁠@@havegrace9111I had a HA 8 years ago and have 5 stents now and shortly due to go in for a bit of ‘maintenance’. My HA was big but damage luckily minimal (I was very fit which helped ironically). Yes , family history here too- I am very fit still- run 5k’s, cycle, lift weights, do body weight exercise, walk more than 10k a day… good diet, very little alcohol- perfect bmi. I’m nearly 50 now. The preamble is to say I agree… it is what it is. Maybe I could reduce carbs/ sugar more and that will help- but everything else is near as advised Med diet etc ; so I get on with living and know that it will maybe get me one day… but a long way off I think; so perhaps you will have a long life too and it will get you EVENTUALLY when you are in your 90’s after a great life… we all have to go.

    I’ve followed keto, lowcarb and carnivore for 35 years.
    I have been extremely active and do cardio/sprints and lift heavy weights. I am a personal trainer, lean, ripped.
    I take all the supplements suggested by PrevMed for heart disease.
    I’m 65…My CIMT clear view says I have the arterial health of an 80+ year old. Plaque burden 9.5, right CCA Bulb 3.2 H, Internal Carotid 3.0 E
    Why? Homozygous Apo E4?
    Is there any help for me to reverse or stop the plaque build up? Please help!

      I'll give an uneducated guess.....maybe your arteries are damaged from too much stress due to working out too intensely.Let them rest.

      What I have read is vit c is important to keep artery supple like they were when we were young. They stiffen with age therefore slowing down the flow. Quick flow is better. I take 1/8 of a tsp cayenne pepper in a little milk. It has the capability of flushing arteries and improving flow and high doses of vit C

    Ariana, how did you find out you were allergic to the contrast dye? Would assume it is from a previous test you had? Reason I ask, is I am scheduled for venogram with IVUS in October, never had contrast dye in my system before. I have two autoimmune diseases and also fear I might have allergic reaction. I am not allergic to iodine, as in Lugols or betadine, but think contrast dye is different type of iodine? They prescribed 40 mg prednisone night before and morning of. Never had prednisone before either - not looking to any of it.

    Is the CIMT what the Life Line Screening service offers? My wife and I (57 and 65) got the carotid artery ultrasound done earlier this year, along with ultrasounds for abdominal aortic aneurysm, atrial fibrillation and peripheral arterial disease. All good, no plaque or things to worry about.

      No, it's different. The screenings you had are good because they give direct information about the arteries in question. Since there is no non-invasive way to screen the coronary arteries, they use a measurement of the carotid artery thickness to indirectly estimate the risk of coronary artery disease. It's not a measurement of plaque, but of inflammation that studies have found correlate to coronary artery disease risk.

      I have had lifeline said i was great then had calcium score which stated i had 170 score
      Then had cmit which should i had 170 but was all hard plaque

    A bit off subject so sorry for this. Is it still worth taking niacin ? Tried to take it, and the flushing was so severe that many parts of my body developed tiny tiny tiny pimples with serious itching for many days after stopping. Statins had too many side effects. So now what ? Thanks.

    Hope you can help clarify my understanding:
    (1) carotid artery is the artery on the neck region. This means these are blood flowing to the brain. Won't it be logical to say that if there is a clot, it is more likely to create a stroke on the brain than a heart attack? How does the clot cause a heart attack if blood is flowing towards the brain?
    (2) when a weak plague breaks on the artery, it was described that blood flow out. Would that be serum from the vasa vasorum? If not from the vasa vasorum, then I would assume that a damage to the endothelium would result in blood flowing into the artery walls instead right?

      So we are talking about plaque build up in arteries of the heart, so, what has been shown is that if you develop plaque in your carotids, your are very, very likely developing plaque in your coronary arteries, that doesn't mean that significant plaque in your carotids is not a risk for stroke, it is as well, a clot in there will go to the brain directly, but we are focusing on heart disease assessment for now.
      So if you have plque in your carotids, we know you have plaque in your coronaries, and is very likely you have similar type of plaques, if unstable, it can rupture and cause a heart attack
      Now the plaque is in the endothelium, the inner layer of the artery wall, damage there causes inflammation that builds up with other substances including some cholesterol but also coagulation factors and platelets,
      Not so much issue with vasa vasorum, those are smaller vessels that provide blood to the artery itself

      A CIMT screening is not done to evaluate plaque in the carotid artery or to determine the risk of stroke. It measures the thickness of the walls of the artery in a segment that is actually free of plaque, to estimate the risk of disease in the coronary arteries. It is an indirect non-invasive screening of arterial wall inflammation that has been found to correlate to the risk of coronary artery disease and stroke.

      @@ExtaTer Those are images and measurements of plaque, the IMT measurement would not be taken in that segment. The standard site we use to obtain the IMT measurement is the distal common carotid artery, and that is specifically because it is usually free of plaque. If the test had been designed to measure the largest plaque, the measurement would be taken in the carotid bulb or proximal internal carotid artery, but that is not the purpose of the screening.

      I live in Ocala and every other year do the Lifeline screening. Am quite sure they do this in Orlando as well, but not sure on the timing. A google search would show you, easy sign up, fairly low out of pocket cost. Dr Brewer commented to one asking the question if Lifeline screening used US, and he said in his experience, they did.
      Also wondering if vascular surgeon offices who do ultrasounds of veins/arteries, offer CIMT.

    so I have a high CAC score, did a stress test and CIMT that was positive for soft plaque on left side
    Cardiologist just kept talking about statins
    I walk 3 - 5 miles a day with my dog, do strength traing 2x and VO2 sprints 3x /week and saunoa (20 min 180 F) 5 days a week.
    BP is 120/80 or lower
    what else can I do?
    I'm listening to whole replay to learn more

    I used Life Line Screening to do a CIMT, but I’m a little disappointed (Aug 2, 2024). Not very much detail. It says my risk is Mild, both left and right says PSV: 11 - 125 cm/s. It seems they are reporting flow obstruction rather than an analysis of the type of plaque. My earlier CAC score is 518 (December 2023). Do you recommend where or who we can use to do a more detailed CIMT?

      @@PrevMedHealth They do use it, there is no doubt. I was lot overseas and doctors there use it as well. I don't mean only cardiologists, but primary care physicians as well know about it. The only thing is that it's not first choice when they need to test for cardiovascular disease. It's still stress test and EKG. CIMT is kind of similar to Calcium CT scan, not first choice.

    I am on Medicare and correct they do not pay for the CIMT test. I am resolute, and convinced my doc this test is important to my treatment, (already had CAC of 167). I had to step up, insist on this test, and manage up to make it happen. The only lab I could find quoted $800, all out of pocket, and I am living on $30k a year. Thats a lot of money, but I guess its a lot cheaper than a grave plot. In the mean time, focused on more discipline diet and exercise, and taking low dose aspirin. I am 68 Years old.

    Carotid Dopplers do not just evaluate flow. They evaluate the plaque and plaque morphology, and use velocities within a stenosis to determine the degree of stenosis that does correlate with direct risk of stroke. They also absolutely do differentiate between calcified and non-calcified, soft plaques. CIMT studies are not even intended to look at plaque. They simply obtain an average thickness of the carotid artery wall in a segment of the artery that is essentially free of visible plaque, and plug it in to a formula, along with the age and sex of the patient, that indirectly estimates the risk of coronary artery disease and heart attack. Two very different exams with different indications.

    CIMT is NOT the ONLY method of detecting soft plaque . CT angiograms can do this in the coronary arteries according to Dr. Google. Search results show that there are many studies and papers showing that this is possible with CT angiograms. I am not sure of the accuracy of each method in quantifying the amount of soft plaque?? CIMT is more desirable due to lower cost, being non- invasive and with no radiation. Any comment on the accuracy of each procedure??

      It might seem like calcium makes sense but that's not the case, some help like nattokinase, vitamin k2, vitamin D3, even berberine, but, none of them are as effective as a healthy lifestyle

      @PrevMedHealth I'm active and am trying to eat right, but it's so confusing to know what diet menu to follow,, one says this, another says that - I'm going to try the low carb diet, but understand that some of the items on that list are not something that's good for cholesterol,, i.e. shrimp. My health store professional says shrimp is nothing BUT cholesterol and not to eat it (she watches you too!)

      That's a clear missconception, dietary cholesterol, like the present on shrimp and Eggs are not a problem, unless You are allergic, almost any diet works, except for the SAD (standard american diet) is about finding what works for You, (You can do the diet long term and improves your biomarkers)

    I just had surgery (endarterectomy) to remove 89% blockage from my right carotid artery. I have had previous TIA. The left carotid has a 69-70% blockage. My heart cath shows normal arteries. What am I to make of this? If you could answer my question I sure would appreciate it. Thank you.

      This is actually a very common scenario. Often those with disease in large arteries such as the carotid, iliac, and femoral arteries do not develop problems in the smaller arteries such as the coronary, renal/mesenteric, and tibial arteries. The reverse is also true.

      Just the neck, but it is usually a reflection of other arteries including coronary, Cleerly CTA Scan is more specific but more invasive

    Unfortunately I can't get a CIMT test here in Australia. Hopefully one day we will have this available . My cardiologist says that it is not clinically approved/significant. I've tried locating a clinic that does this test but have drawn a blank so it looks like no-one has cimt available in Aus?.

      Drink a shot of olive oil extra virgin if you want to do it naturally and eat healthy, don’t smoke, exercise or you can have surgery to remove it or have a stent placed if needed
