Although it's rare and often presents with more than just palpitations, pheochromocytoma is a notable disease that in retrospect I should have included in the palpitations framework.
You can ask the patient to tap on the desk as per the rhythm of his felt arrhythmia if arrhythmia is intermittent.

Pheochromocytoma?
I had panic attack 3 years ago, HR jump from 60 to 130, SOB, I thought I was going to die. I run out in middle of the exam, the professor said she thinks I had panic attack. This panic attack continued a few months. Terrible
Hello, I have tried many suggestions for PVC, but I am still suffering them for many years. Is there anything you can suggest that I may try? Or has anyone found the solution to PVC, as it is truly life altering and debilitating.
I can't give specific, individualized medical advice on here, but if you live in a part of the world where you have access to electrophysiologists (cardiologists further specialized in the treatment of abnormal heart rhythms), a PVC ablation procedure is relatively straight-forward, and something that you could speak to your own physician about.

How about Lipid profile, Magnisium test,hgb& thiamine test?
Hgb is part of the workup of unexplained sinus tachycardia - thanks for pointing out its omission!
If ECG or cardiac monitor were to show that the patient's palpitations were due to premature beats or a paroxysmal tachycardia, then checking a magnesium level would be appropriate.
I would not consider a lipid panel or thiamine level to be part of the routine workup of palpitations.

, But in my opinion a poor country like ETHIOPIA Magnisium difficency &beriberi are common so i Shall consider those test to rullout the causes of palpitation routinely .
