    My memory isn’t what it was, and I haven’t fact-checked it, but I think I remember Ivor Cummins quoting The Framingham Study saying that CVD risk doesn’t increase until LDL hits 7.8mmol and, even then, only if accompanied by HDL of under 0.8. This work reinforces that. The arbitrary reduction in LDL targets over the years is despicable and unnecessary.

      The reduction of LDL targets reflects the evidence from numerous trials showing significant reductions in major adverse cardiovascular events with cholesterol lowering through diet/lifestyle/drugs. This is hardly either despicable or unnecessary. It also reflects the gradual realisation that 'normal' isn't always the same thing as 'optimal'.

    Thank you Dave Feldman!!! A few minutes after watching this I started crying for joy. Three years on meat only diet out of necessity. I am very lean and all my labs changed just as predicted by your model. This is extremely good news as I can’t eat anything from a plant without feeling awful for days.     Hi, I went low-carb in 2019 and watched my LDL Skyrocket. I am 58 years old and always had normal to low LDL until going low-carb. Over that time I have gone more strict with low-carb- going from very low-carb to essentially no carb (now doing lion diet).
    I had a calcium score done on December 1 and it came back zero. Going into it I thought I would have at least some plaque, though I have always been athletic and active, i’ve definitely slowed down over the past decade after a series of shoulder surgeries back in 2014/2015 my main form of exercise has been primarily walking just recently, over the past three months started lifting light weights a couple times a week. So I was somewhat surprised that I would have all zeros especially in my late 50’s especially after hearing about autopsies of both Vietnam era and Iraq soldiers, showing heavy plaque build-up in soldiers in their early adult years (late teens to early 20s).

    Dave, I recall sitting in Carl's front yard, about midnight after a VIP event discussing crowd sourced research and how it could be done. I am impressed beyond belief how far that conversation went. Kudos my friend.

    Good on them for doing this, a couple notes however:
    *1. 4.7 years is not long enough to see plaque development for a multidecadal disease.*
    *2. The mean LDL-C is 123 mg/dL for the second group, that is still above recommended levels in conventional medicine. The study should've compared to those with LDL-Cs less than 100 mg/dL.*

      @_RumpledForeskin 1. It is long enough to establish LDL levels above 123 mg/dL are not a meaningful factor in causing atherosclerosis in LMHRs on a strict keto diet. Pretty narrowly defined (and rare) test group, but it does seem to demonstrate that claim. Now it needs to be expanded and reproduced by others for it to gain confidence it wasn't a one-off result.
      2. True and both groups still grew plaque.
      3. I see no all-cause mortality results, perhaps because the groups were small, not elderly, in good health and this was for only 4.7 years duration. But there are nutritional controversies beyond LDL when it comes to strict keto diets. There may be other non-atherosclerotic all-cause mortality risks inherent to strict keto that arise over much longer periods of time. The whole point is to live a longer, healthy life. Getting rid of one risk factor simply to unknowingly adopt another would defeat the purpose.

      If I may ask, how have you avoided your doctor trying to foist a statin onto you? I’m a new carnivore (7 months in), and I anticipate my doc may try to prescribe a statin for me at my next bloodwork results. I follow this subject with great interest too! I’m a big fan of Dr. Ken Berry. 🙏

      ​@@susangrande8142I ignored the prescription when I was young. In my older (wiser) years I shared my research with my Doctor and when he still wasn't convinced I asked for a CAC. He's left me alone since.

      ​​@@tserevenadlast 4 years 18/6 keto as a baseline. Unfortunately I only have full lipid history for past decade or so. My bloods from 30 years back only included "good" & "bad" cholesterol so unfortunately I don't have the full picture that far back. I was also lean then, training hard for brown belt grading at the time of my first very elevated LDL. My diet varied over the years though I generally tried to eat real food, with some excursions into SAD especially when stressed out on challenging work projects.

      I remember attempting to review Dave’s work with my cardiologist. He would have nothing to do with it and suggested I become vegan. I don’t see that doc or any cardiologist any longer. I’m LMHR!!

I've been following you and the LMHR study/info/seminars for awhile now and just reached one year on low carb, with trending LMHR lab results at the 10-month period.     Thank you for your dedication and honesty on the LDL and risk in LMHRs. Because of people like I held off the use of statins which my doctor keeps prescribing because my LDL jumped to the 250s when all other health indicators such as weight reduction, blood pressure, stamina, were looking fantastic. It did not make sense so and I refused to take the statins. I took two CAC tests in the last three years and got scores of zero. I understand there could still be soft plaque-this study somewhat alleviates some of my concerns. Thank you.

      New information that 1 in 1000 people with specific traits and low BMI have not experienced progression of heart disease in a short period? How is that some revelation and how will it help the average person decide on what diet is healthy to them? Double heart disease events recorded in another study on low carb on not some specific LMHR, how does this study refute any of that?

    Thanks for your hard work. Awesome outcome and great information. I started Keto 5 years ago and a LMHR. Just had a CAC and echocardiogram. My CAC was 260 no major blockage. I however came to eating a keto diet for uncomfortable inflammation and pain. Was athletic until my early thirties and my health spiraled out of control for 15 years until strict low carb. I are high carb and a lot of seed oils. I also was a sugar addict. I am waiting for the final results of the LMHR study for those who had plaque to see progression. But for me my CAC score has made me more focused on keeping my blood sugar low. I slipped up a little this year. Back on track and eating super strict low carb keto with meal timing.

    Congratulations Dave! At first to be honest, I was disappointed that the lmhr wasn't significantly better off than control, but then I see how closely matched the controls are - right down to their blood pressure and, hscrp, and a healthy hdl to trig ratio. The biggest difference I was was BMI, it looked like it barely put the Miami cohort into the overweight category. Also, so many of the Miami heart group were on lipid meds, you would think that would be quite helpful for their scores!? and yet there was no statistical difference. LMHR's rejoice! And get your cac scans people

    Thank you for sharing this information and talking about it so that anyone can understand it. It takes a lot of courage to ask questions where others won't but to gather data to try to answer those questions is even harder. It's inspirational to see everyone who contributed to this study working towards obtaining data that well undoubtedly help humanity as a whole and lead to a lot less unnecessary suffering.

    Thanks, Dave, for your perseverance and dedicated pursuit of understanding the atherosclerotic risk from sustained high LDL-C in those who are metabolically healthy. Compelling baseline comparison data that, in the very least, should spark questions among lipidologists and cardiologists.

    Congrats, Dave! This must feel so good to get such exciting data after such a long-term effort.
    I'd like to cover this in my next video (briefly) about LDL-chol/apo B and ASCVD, because it's quite noteworthy. Question: do you have historical data of the keto particpants of their LDL-chol levels for the 4+ years they were following the keto diet? How about apo B levels? Is it fair to say that the difference in apo B is a lot smaller than that of LDL-chol?
    Congrats, again! I am hoping this will give you a boost, also in terms of funding, to continue this line of work. The longitudinal data should prove highly informative!

    I have been following a Keto/Ketovore diet for the past 4 years. According to my total Cholesterol and LDL cholesterol has been high for the past few years. Would my last two results put me in the LMHR category? I'm 5'6" at 168 pounds with a lean muscular build. Below are my last two test from the VA and my regular health care provider. My last few test have been similar. I feel much better on a keto diet and even better when I'm on strict carnivore. Thanks in advance for your input.
    July 2023:
    Total cholesterol: 474
    LDL: 357
    HDL: 43
    Triglycerides: 118
    September 2023
    Total cholesterol: 455
    LDL: 369 (VLDL: 41)
    HDL: 45
    Triglycerides: 185
    Also I have recently been referred to a cardiologist to take a further look and my echocardiogram was normal and my blood pressure has never been high. I will be getting a calcium score as a follow up.
    I'm 45 years old, forgot to mention.

      Your HDL is too low and your trig's too high to group you as a true LMHR. Definitely do a CAC to check your arteries. I had a somewhat similar experience that my trigs didn't go as low as others on a keto diet and found that adding 3 servings of fatty fish (salmon, mackerel and sardines) to my weekly diet helped to finally lower my trigs. It seems that Omega-3 fat is very important for some people to control their trigs. Another thing when checking your trigs is to make sure that you fast for 12-14 hrs before you have your blood drawn so that trigs are not artificially high due to the effect of your last meal

    I think it is time for all to stop calling LDL and HDL "cholesterol" as though they were types of cholesterol instead of carriers of it. Malcolm Kendrick says "You wouldn't call a car a person because it carries people around". It is time to stop perpetuating this misunderstanding.

    Amazing work that Dave and his team have done, 4+ years on the Ketogenic diet LDL 3.6 mmol/ High HDL low Trigs. I recently discovered that I have a calcium score of 277 and it has been going through my mind that it may be the ketogenic diet even though I felt the best I have ever felt so I did some research and had a test for Lp(a) and discovered that I have a score of 277nmol/L so this is the smoking gun not the diet. Now I am unsure which way to go as all the information I can find suggests lowering my LDL !

    So essentially if you are LMHR, LDL is not an issue but conversely if you are not LMHR, then work to get your LDL down. I have high LDL but also low HDL (40-ish mg/dl) on keto but also CAC >200.

      We already have quite a few studies demonstrating masks and lockdowns had no effect in reducing COVID transmission.

    It takes 30 years of exposure to LDL. Fundamental error is the period of time of the study
