The BEST way to Manipulate (HVT) the Thoracic Spine & Ribs

    John Gibbons is a registered Osteopath, Lecturer and Author and is discussing how to assess (and then subsequently manipulate) the spinal motion of the thoracic spine using Dr Lovett (1903) and the laws or principles of Fryette (1918), John also manipulates with a HVT / Grade 5 technique to the thoracic spine and rib cage. Some of these techniques can be taught but only to skilled therapists and manipulations plus more is taught on the Spinal course at the University of Oxford.

    Hi! sir i have a question in bout in first techinique about turning the hands while pushing down ,is the left also turning ? i only saw that that right hand only turning or the the two hands turning both turning?

    What can be done of lower thoracic vertebra is forced forward and can’t flex forward, forcing ribs forward and up and upper lumbar spine is forced out so tight won’t release.
    No position bearable because effecting both lower and upper body.
    Seen various ppl. Nobody can release it.
    Any suggestions gratefully received.

    I really addicted to watch that kind of Posterior Thoracic Thrust Manipulation. May I ask? Aside from double thenar and cross-handed pisiform contact, can you use Hand contact with fingers interlocked, (similar to CPR Hand Placement for chest compressions), to adjust the thoracic spine?

    Thank you so much for your wonderful teaching . Is it allowed to preform these techniques on T11 T12 area as well? (The difference on the angle of fhe facet joint...).is this possible to use the same technique on the 1 rib as well?thanks in advance for your answer.

      Lower thoracic maybe but not the first 3 ribs as the technique I use is different, regards JG

    Excellent teachings, I find my self tuning in to your videos (morso than others) because of the details you provide. One question: Are there applications where the direction of rotation (and thus hand placement) would directly correlate to the direction of subluxation of a particular vertebrae?
    If so, I would love to see a video explaining how that works and how to identify the directions the vertebrae need to go. My understanding now is a twist in either direction aligns subluxations no matter the direction of the subluxation, is that correct, or are there applications where the directions matter?

    Great video!
    I just have a question: in the first mobilisation, do you place your hands close to the spinous process ( over the vertebral lateral process) or are them on the ribs? Same for the tird mobilisation.
    I couldn't understand weel.

    John, I am suffering from my rib cage lifts , due to wrong setting for neck traction last year ( 39 p, 0 degree) , I feel pop up near neck on left side. and chest painful at that night , now I saw a few Chiropractors do not work and move back my rib cage to original position, I have breathing difficuty , How can you fix it.. Thanks.

    In the last manipulation why you stand behind the patient? The line of joints is going in different direction so isn`t that a compression of joints? Im refering to Mulligan where mobilisation is in sitting and direction is to "eyes".

    John, love your videos, very educational. I'm a PT and sometimes utilize the mobilization techniques for my patients. I have a question regarding the last approach you have shown. I was taught that because of the direction of the spinal processes in the thoracic spine (kind of like shingles on the roof), it's better to approach from the other direction - standing at the hips and applying force towards the head/ mat. Could you comment. Thank you

      You are not on the spinous processes - you use the angle of the ribs to cavitate the costo-transverse joints - regards JG

    At last someone concentrating on the area where I am in so much pain! Can I ask is there a cure for this particular part of the body or is it going to be endless trips to the chiropractors for life? I say this because my husband has a chiropractor who manipulates his lower back for 20mins, he feels ok for a few days then it starts back up! Thank you

      Hard to say without looking at your husband, manipulating the spine is great but needs to have other modalities - exercises, soft tissue techniques - diet advice etc, etc regards JG

    Hi there! Osteopathic student right now and just curious, but is it not necessary to get a diagnosis of these vertebra prior to treatment with springing/thrusting techniques like HVLA? I'm just wondering if it's safe and fine to do these techniques without being very specific about diagnosis and the levels we're at. thanks!

      You can vary the techniques so you mobilise the rib cage rather than manipulate - if you are an osteopathic student you will be shown these and the tutor will probably use my videos to teach you - regards JG

    Hey John, do you find these better then dogs? or is there any reason you would use a dog over these (apart from the fact they can't lie prone for whatever reason)? and what about 1 and 2nd ribs? TIA for taking time out from the Aston :)

    Thank you for this video. I have a question: 3 months ago I went to see an Osteopath who did manipulation on my upper lumbar spine and ever since that session, I'm unable to sleep. The minute my body drifts to sleep, my heart would start racing and I end up waking up. I hardly get to sleep as a result. Is this a normal response to the treatment? Is this something that can be reversed?According to the Osteopath, she worked on the area that has an important influence on the respiratory diaphragm to which the heart is connected. .She said had increasing the motion of my diaphragm and upper lumbar spine which would have had an influence on any soft tissue pulling on the connective tissues surrounding my heart!

      Thanks for the message and I doubt if the Osteopath caused the problem you are experiencing. I have seen thousands of patents and I perform the manipulation to the lumbar spine and never have caused those symptoms. Maybe see a Doctor and see what they suggest, regards JG

      Typically a High Velocity Thrust (HVT) is a common term osteopaths use - Chiropractors typically call them an Adjustment and Phsiotherapists call a manipulation a Grade 5 technique - basically all the same apart from the terminology - regards JG
