Great topic! Getting to/from international cruise ports is such a hassle that we no longer do 7 day cruises. It's a 12 day minimum for us. We're currently booked on 16, 22, and 32 day sailings! Though, I wish there were dentists onboard in case you crack a tooth.

I couldn't ever retire on a ship because I am now solo and disabled, but even when I did cruise for a short time in the past, I would gain a pound for every day I cruised. Therefore, I don't think my waistline could ever handle me staying on a cruise long-term even if I wanted to. You two must either exercise and/or have much more self-control when allowed to eat whatever you want, so good for you.
I think it would be important to not put all your eggs in one basket - find multiple ships you like on multiple cruise lines and switch between them based on itineraries. Yes loyalty programs are important but everything can change with little notice. I really think paying attention to the trans atlantic and pacific cruises and using them for transportation between longer land stays and explorations would be a great strategy since the trans cruises are often cheaper than airfare between the same locations. I'm planning a little version of that for vacation by taking Amtrak down to Fla and then a cruise through the Panama Canal followed up with Amtrak home.

If you are only going to eat in the buffet, yes you are going to get bored with it - branch out and try the other food options. It is the same on any cruise ship, there are many different eating options, other than the buffet.
Try to plan for series of positioning cruises and come to Asia. You can't say to have retired on cruise ships without traveling around the world. There are also actually Japanese cruise ships that does 3 months long voyages and travels around the world. And surprisingly isn't that expensive. Just have to be selective on which ports to visit since tour excursions isn't included.
I think a take away from what you are saying here is you need to research your cruise ship as well as your itinerary. Yes the mega ships have more dining venues but they also have more people to fill in those dining venues. We go on cruises at least once a year if not more and we always have friends and family asking us about helping them to picking a cruise, but you really need to know what you expect on the cruise and what type of activities you like, want to live at amusement park, go on a mega ship, want to hear about the history of place you are going to visit, go on Viking, etc. some people hear about amazing buffets, games, rides and shows and they expect it on all cruise lines. I like the more elegant decor Holland America has, I am ok with the repetitive buffet for breakfast, because I eat dinner the main dining room, dinner is going to be a more gourmet meal. I’m okay with paying yo have laundry done. Because at home I am semi-retired so I still work, still clean the house and rushing around. But I want a vacation where I am not cleaning bathrooms, making beds and cooking. Even get my laundry done as well. I always say where can you go on vacation get your hotel room, your meals and your transportation to different cities for about $100 per person per day? (More now unless you find a deal) Research your cruise line as much as the itinerary it sail.
What do you do about stuff like dental/medical appointments? Prescription drugs? I know that there are medical services on the ships, but these tend to be only for emergencies if you’re a passenger.

Most Carnival ships especially the new ships (Excel class - Celebration, Jubilee, Mardi gras) or retrofitted ships (Venezia, Firenze, etc.) and old refurbished ships DO NOT have laundromats or even a borrowed iron to press your dress clothes. They will make you pay for laundry and pressing for each piece of the article.
What about medical? As we get older we often need more medical, not less. I spent 3 weeks on a cruise and was surprised at the high cost (I didn't need any thank goodness)

Check some of their older videos. I believe they said it's been a process in one of their early videos. Some things went in storage. I don't recall if they have family, but I know some other youtubers mentioned staying with family during the holidays etc then back on the road. I recall they had a video on staying in long term hotels or airbnbs in low cost countries and taking advantage of hotel points and rewards for free nights.
Missing family, not having regular health providers, not having good internet are enough reasons but I’m someone who gets very bored on sea days so this is not for me.
Big fan of the channel but I think I am turning on these two. They aren't "retired", they are just on an extended (years long) vacation. They earn money/work with these RUclips channels. They are as "retired" as Bourdain or any other travel vlogger. Also, if you think about it, this lifestyle is for the extremely selfish. We have nieces and nephews who are growing up and we want to be around for that! It might sound like sour grapes but I am simply not wiling to look at this lifestyle with awe and wonder without asking serious questions. With that said, I still watch the channel 😀

Newer ships don't tend to have a library. That's why I like the older ships.
