That look that Carmela gave Tony was priceless. It looked like she wanted to say Jesus Christ Tony! Goddamit this is not about you and your vindictive mother or your depression! Get over it!
She gave him that look because she knew Tony was setting himself up to go back into therapy with an attractive woman that he would inevitably attempt to seduce. When she was gone from the room Tony made that apparent by the looks he gave her, as he was attempting to charm her nonverbally (not shown in this clip). Funny because he never acted this way or came out to AJs therapist this way. Carmella saw he was attempting to project the same fantasy situation he had with Melfi onto this new therapist and got jealous. Great scene that revealed Tony's character and motives, which occurs a few scenes after Melfi had kicked him out of therapy.

It's because she's an inadequate therapist, she bizarrely sees Livia's outright sadism and spiteful nature as BPD symptoms rather than more obviously NPD symptoms which even Janice later recognises.
No it would not have. Aj wanting to join the military was actually a fantasy brought on by subconscious programming from the things he was seeing on television and reading on the internet. Just like how he took the commercials and advertisements about depression and sleep deprivation and used that to justify his lack of motivation to do anything. On top of that fact he was just emulating his father. Carmella picked up on it and that's why the fight with Tony happened just after the suicide attempt when Tony tells her he is now depressed. In season 6 AJ changes his career aspirations every other episode. We also see Meadow have a similar crisis at the end of the series. It's clear they both have become aware of their own privilege in life and deep down resent it, which in the end serves to show that Tony's sacrifices for his family were in vain. This show uses televisions and art to show the viewer the kind of fantasies and motivations going on in characters heads, and how what they see also projects certain ideas into their own subconscious. We see this throughout the entire show with Tony and his fascination with war, cowboy movies and history documentaries; and with Carmella and her fascination with Renaissance art.
The sad thing is Livia is clearly NPD as Janice basically said in season 6 if my memory is correct, so if he got this "diagnosis" from Melfi it's pretty inaccurate. There are unofficially recognised "subtypes" of BPD where the impulsive subtype is very narcissistic/histrionic but otherwise they are actually clearly distinct personality disorders and a competent psychiatrist would know that, BPD sufferers are capable of empathy and often do want to work on themselves, they just have a very insecure sense of self and a pattern of impulsive self destructive behaviour. NPD is far harder to treat and they will very rarely actively seek out therapy in the first place (because they will usually refuse to recognise anything is wrong with them)
