    Reminder: Carotid arteries should be auscultated (listened to) before palpated. If there is a bruit, we don't palpate because you don't want to accidentally break off the buildup attached to the artery (potentially causing a stroke). Great video overall! Cheers!

    This helped me so much! My first assessment literally took me an hour 😂😂 Our instructor doesn't really go into detail about assessments and we're all still so lost about it but this video was awesome!!

    Huge help! I start clinicals tomorrow, haven't done test outs on head to toe assessments yet, however I know this will be a huge part of clinicals. Thank you for clarifying this in a no nonsense-this is how I do it-approach. This is exactly what I needed.

    I'm in my first semester in the ADN program at my school and will be tested on this next week. I was actually pretty nervous because our instructors kind of leave us to our own devices and we have to figure things out on our own (unless we ask questions), but this video made it so much easier.     I'm an LPN (for about 12 years now) currently in the process of getting accepted into an RN program. They are making us do skills check offs, one being a head to toe assessment, which isn't something that is all that common for LPNs to do. I have been a nervous wreck, going over landmarks in my sleep!! This is the most helpful video and info I have found yet. Thanks!!

    I've seen an RN give a very poor assessment after a pts fall, shortly afterwards , I had to begin neuro checks , and you were very thorough , she basically looked for blood on the scalp. I'm learning so much from you thanks!

    make sure to know which questions to ask, some you can clearly marked off. I totally asked a post menopausal 77 year old woman when was her last period it was so embarrassing . she laughed though so yeah that happened.

      That's funny, but don't feel bad. I asked a lady who had gotten hit in the eye with a remote controller her pain scale; and then asked her where the pain was located. Totally didn't think that one through...

    My biggest like for this vid is the way you make this general assessment flow. I think that's sometimes the hardest part of learning all of the individual assessment separately. There's no flow...and every prof says, "you'll develop your own style and sequence". But it's nice to see (or hear) a nurse putting it together, Profs don't repeat their expected flow more than a couple of times. This is helpful to practice to and develop a timely routine for a H2T assessment. I also love that you are very verbal...it helps to absorb info even when I'm busy with housework or something and can't watch the screen. Thanks Nurse Bass!

    I dont know what to say. Went to skills lab a couple days ago, the professor demonstrated the head to toe assessment and I left skills feeling overwhelmed and depressed, lol.     Great information! I always started my assessments backward, starting at the toes. For some reason it always felt more comfortable for me to start that way ~ I would start with checking pedal pulses, heels for ulcers, etc and work my way up. It seemed to put patients at ease. Just my personal technique ~ Of course, if I was doing a focused assessment such as a mental status or pulmonary assessment, I would start at the point of concern.

    I have my first head to toe assessment in my clinical today (which would be in 2 hrs) and after watching this video, I definitely feel more confident for clinical. In other educational videos and books, it gives alot of overwhelming info (it does seem like your every assessment should take 30 mins every time haha).     This is a very generalized 'Head - To - Toe' Assessment video, however remember your lecture in nursing school regarding about certain areas such as Edema's etc.
    You have to remember that in NCLEX it's mentioned that you have to use the 4 basic techniques for assessment which includes (IN THIS ORDER) Inspection, Palpation, Percussion, and Auscultation. This order is important to remember as it is different in the abdominal area (the order changes into Inspection, Auscultation, Percussion, and Palpation). The rationale for that is the patient might have abdominal pain and if you palpate right away bowel sounds will get disturbed, or general assessment data might not be accurate as the patient will complain 'everything hurts',so palpate last in the abdominal area.
    Second, for checking Edemas press down on the skin, remember it's graded to either +1,+2,+3, or +4. Each grade has a significance in terms of measurement (Or how bad the patient's edema is), which in this case +1 is 0-2mm , +2 2-4mm, +3 4-6mm +4 more than 7mm depression.
    Every nursing school is different as to how they teach these, but these are the very basic all nursing student's should know. Remember that when your studying, every knowledge your obtaining will be applied for a person who will be under your care.

    This was a fantastic video!! Very thorough and helpful. My program requires we check for edema on the face and extremities however you covered everything else really, really well. Thanks so much for your time in putting these videos together:)

    Thanks so much! I am just starting my second semester and this is the first chapter we are doing. Seeing it done after reading (and before practicing in lab) is making it make more sense (it's a loooong chapter!).
